A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF AZUSA ALLOWING CERTAIN CLAIMS AND DEMANDS
AND SPECIFYING THE FUNDS OUT OF WHICH THE
SAME ARE TO BE PAID.
The City -Council of the City of Azusa does resolve as
follows:
SECTION 1. That the following claims and demands have
been audited as required by law and that the same are hereby allowed
in the amounts and ordered paid out of the respective funds as
hereinafter set forth:

A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF AZUSA FINDING AND DETERMINING THAT A CERTAIN
CITY EMPLOYEE IS NOT INCAPACITATED FOR THE PER-
FORMANCE OF DUTIES AND SHOULD NOT BE RETIRED FOR
DISABILITY.
(Section 21023.6 Government Code)
WHEREAS, the City of Azusa (hereinafter referred to
as Agency) is a contracting agency of the Public Employees'
Retirement System;
WHEREAS, the Public Employees' Retirement Law requires
that a contracting agency determine whether an employee of
such agency in employment in which he is classified as a
local safety member is disabled for purposes of the Public
Employees' Retirement Law and whether such disability is
"industrial" within the meaning of such Law;
WHEREAS, an,application for disability retirement of
LLOYD R. JACOBSON, employed by the Agency in the position
of Police Officer has been filed with the Public Employees
Retirement System; and
WHEREAS, the City Council of the City of Azusa has
reviewed the medical and other evidence relevant to such
alleged disability;
NOW, THEREFORE, BE IT RESOLVED: That the City Council
of the City of Azusa does hereby find and determine that
LLOYD R. JACOBSON, is not incapacitated within the meaning
of the Public Employees' Retirement Law for performance of
duties in the Police Department.
The City Clerk shall certify to the adoption of this
resolution and shall cause a certified copy thereof to be
transmitted to the Public Employees' Retirement System.
Adopted and approved this 18th day of August, 1980.

A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF AZUSA A1ENDING PESOLUTION NO.
5396 DESIGNATING STOP AND SIGNALIZED
INTERSECTIONS.
THE CITY COUNCIL OF THE CITY OF AZUSA DOES
RESOLVE AS FOLLOWS:
SECTION 1. Resolution No. 5396 adopted by the
City Council of the City of Azusa on Ilay 6, 1968, is
hereby amended by adding the following intersections and
entrances to intersections at which traffic control devices
shall be installed and the driver of any vehicle shall be
required to observe any limitations or restrictions when
traffic devices are erected giving notice thereof:
"(agggg) 12th Street at Vernon Avenue
"(hhhhh) 13th Street at Vernon Avenue
"(iiiii) Duggan Avenue at Gladstone
Street."
SECTION 2. The City Clerk shall certify to
the adoption of this Resolution.
ADOPTED AND APPROVED this 18 day of August , 1980.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF AZUSA ALLOWING.CERTAIN CLAIMS AND DEMANDS
AND SPECIFYING THE FUNDS OUT OF WHICH THE
SAME ARE TO BE PAID.
The City -Council of the City of Azusa does resolve as
follows;
SECTION 1. That the following claims and demands have
been audited as required by law and that the same are hereby allowed
