HomeMy WebLinkAboutResolution No. 95-C138�J RESOLUTION NO. 95-C138 11 A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF AZUSA AUTHORIZING THE INTRODUCTION OF THE "HOLIDAY SEASON UTILITY REBATE PROGRAM" AS A REVENUE -RAISING MEASURE WHEREAS, during the hearings on the FY 1995-96 Budget, staff proposed several possible revenue -raising measures aimed at balancing the budget, and WHEREAS, Phase 2 of the proposal is to provide a financial reward to utility customers who submit valid receipts for purchases from Azusa businesses, NOW, THEREFORE, BE IT RESOLVED that the City Council agrees to introduce Phase 2 of the revenue -raising campaign to be known as the "Holiday Season Utility Rebate Program" with proposed rules as follows: 1. The program begins on November 1, 1995 and will end on January 15, 1996 and is open only to City of Azusa utility customers and their households. 2. By submitting the appropriate receipts the utility customer will earn a rebate award on his or her Azusa purchases as follows: (a) For submitting original receipts totaling $ 49.00 in purchases from Azusa businesses the customer will receive a credit of $1.00 on the following month's utility bill; (b) For submitting original receipts totaling $ 99.00 in purchases from Azusa businesses the customer will receive a credit of $2.00 on the following month's utility bill; (c) For submitting original receipts totaling $149.00 in purchases from Azusa businesses the customer will receive a credit of $3.00 on the following month's utility bill. 3. Receipts must be dated between November 1, 1995 and January 15, 1996 and must be submitted no later than January 31, 1996. Only one rebate will be paid per utility customer/household. 4. In order to receive credit, receipts can only be submitted with the normal utility payment, either by mail to the normal payment address or in person. If the customer makes payment/redemption in person, a utility representative will cancel the receipts and, upon request, will return them to the customer. If the customer makes payment/redemption by mail, the receipts will not be returned and the utility department will not be obligated to consider any such requests. If the customer anticipates that proof of purchase will be required for warranty purposes, etc., he or she should make a photocopy for his or her records. The utility department will not be obligated to furnish such photocopies. 5. Receipts submitted for credit must clearly identify the name of the Azusa business that issued it and must be clearly dated between November 1, 1995 and January 15, 1996. Receipts that do not bear these two required identifiers, and/or are not originals or otherwise do not appear to be bona fide, will be discarded without notice to the customer who submitted them and no credit for them shall be given. 6. Rebate awards will be made only in the form of Azusa utility bill credit. No cash rebates will be made. 0 0 7. Azusa Avenue, Azusa, California 91702. While every effort will be made to answer all inquiries, a personal response cannot be promised.
