1. Mayor Roe -ha - request for $200 sponsorship for the Azusa High School Wrestling Team.
2. Councilmember Macias - request for certificates of recognition for outgoing members of the Personnel
Board and Planning Commission, Mrs. Bonnie Guadagnino and Mr. John Dierking, for their services to
City. Discussion regarding recognition for outgoing commission members.
3. Recreation and Family Services Recreation Supervisor Gonzales to provide an update on the Recreation and
Family ,Services Department Holiday Programming.

2. HUMAN RESOURCES ACTION ITEMS. RECOMMENDED ACTION: Approve Personnel Action
Requests in accordance with Section 3.3 of the City of Azusa Civil Service Rules and applicable
Memorandum of Understanding(s).
3. ACCEPTANCE OF IMPROVEMENTS FOR TRACT 63336-01— MAPLETON. RECOMMENDED
ACTION: Accept the improvements required for Tract No. 63336-01 as complete; take the water, sewer
and storm drain systems into the City maintained systems; find that the private streets have been completed;
accept the monumentation of the Tract as complete, and transfer the improvement bonds to a warranty bond
and release the monument bond.
4. THE CITY TREASURER'S REPORT AS OF OCTOBER 31 2008. RECOMMENDED ACTION:
Receive and file the report.
5. APPROVAL OF FINAL PARCEL MAP NO. 63746 FOR THE PROPERTY LOCATED AT 547 E.
TENTH STREET. RECOMMENDED ACTION: Adopt Resolution No. 08-C110, approving Final
Parcel Map No. 63746.

PUBLIC HEARING — CONSIDERATION OF AN AMENDMENT TO THE FIVE-YEAR
IMPLEMENTATION PLAN 2005-2009 FOR THE MERGED.CENTRAL BUSINESS DISTRICT
AND WEST END REDEVELOPMENT PROJECT AREA RECOMMENDED ACTION: Open the
Public Hearing; receive testimony, close the Hearing. Approve the proposed Amendment to the Five -Year
Implementation Plan 2005-2009 ("Implementation Plan") for the Merged Central Business District and West
End Redevelopment Project Area ("Project Area"), which adds streetscape improvement projects.

The City Council of the City of Azusa met in regular session at the above date and time in the Azusa
Auditorium, located at 213 East Foothill Boulevard, Azusa, CA, 91702.
Monrovia Mayor Robb Hammond addressed Council presenting a plaque to the City of Azusa and its
residents in appreciation of the support from the Azusa Police Department for the City of Monrovia during
it's time of trouble.

Mr. Mike Lee addressed Council with comments as follows: prayers and well wishes to Linda Rocha and M. Lee
Mayor Rocha; expressed concern for fire victims; Veterans Day observation; Food Banks need food; good Comments
luck to candidates in the March 96 election; support local businesses; and to pray for troops.
Mr. Victor Flores addressed Council expressing his disappointment regarding the Homeowners Assistance V. Flores
Workshop -held at the Senior Center and requested genuine aid for people in Azusa who are losing their Comments
homes; he stated the workshop was misleading.
Mr. Nick Rosales addressed Council expressing his frustration with the workshop held regarding home N. Rosales
foreclosures and relayed his experience at the workshop. He stated that there is a need for proper forms Comments
and that he is willing to conduct a "how to" free workshop on loan modification. He requested that the city
provide him with a facility and copier in order that he may help people.
Mr. Mark Smith, a proponent of the Gold Line, addressed Council in favor of the City of Azusa's support M. Smith
for the Gold Line extension. Comments
Mr. Art Morales addressed Council commenting on the following: Homeowners Assistance Workshop; the A. Morales
Veterans Monument in City of Irwindale; concern for fire victims and wished all a Happy Thanksgiving. Comments
Mr. Dennis Willut addressed Council with comments as follows: a crisis at 741-743 Soldano; Azusa Light
and Water access to a utility pole/locked panel box; Cable Television; scarred landscape; broken irrigation D. Willut

REPORTS, UPDATES COUNCIL BUSINESS AND ANNOUNCEMENTS -STAFF Rpts/Updams
City Manager Delach expressed appreciation to City employees who participated in the Azusa Rocks U, City Mgr
Earthquake Exercise on November 13, 2008. - Comments
Moved by Councilmember Gonzales, seconded by Councilmember Macias and unanimously" carred to Sponsorship
approve sponsorship in the amount of $100 each for two students of Azusa High Drama Club to go to New Two Drama

Councilmember Gonzales praised the Azusa City Employees for the excellent American Shake Out Drill. Gonzales
He asked about a contingency plan for Mountain Cove if the bridge goes out. City Manager Delach Comments
responded stating that there is high ground area of which residents will be directed. Mr. Gonzales wished
all a Happy Thanksgiving from himself as well as the Rocha Family from Mayor Rocha.
Councilmember Macias requested that staff look into the bollard lights which are out at Rancho Park; to Macias
assist American Little League in repairing their lights; look into the feasibility of expanded the bathroom Comments
facilities at Zacatecas Park and/or possibly placing it in the CB' for next year or following year. He
announced that he is the Treasurer and Secretary of the Gold Line Joint Powers Authority and with the
passage of Measure R, they will work towards getting the Gold Line extension from Pasadena to Azusa.
He talked about his tour of the waste management facility and their long term goals; the disappointment of
the mortgage workshop held on November 126 on Saving the American Dream, and a new workshop he
and Library Director Tovar are working on, and inquired about the payment to California Resource
Connections in compliance with the MOU. City Manager Delach responded that they will issue funds as
they were awaiting a final report. He wished all a Happy Thanksgiving.
Councilmember Hanks expressed best wishes to people affected by the fire and to all a Happy Hanks
Thanksgiving. Comments
Mayor Pro -Tem Carrillo wished prayers to Joe and Linda Rocha, the fire victims, and his father, reminding Carrillo
all of the bad air quality. He wished all a Happy Thanksgiving and reminded all that kids will be out of Comments
school during the holidays.

PUBLIC HEARING - ORDINANCE AMENDING CHAPTER 30 AZUSA MUNICIPAL CODE AND 2008 Fire Code
ADOPTION OF THE 2008 FIRE CODE OF THE LOS ANGELES COUNTY CODE Prop Ord
Mayor Pm -Tem Carrillo declared the hearing open. City Clerk Mendoza read the affidavit of proof of Hrg open
publication of notice of said hearing published in the San Gabriel Valley Tribune on October 30'° and
November 6'". 2008.
Economic and Community Development Director Christianson addressed the issue stating that this is a Com Dev Dir
revision to the Azusa Municipal Code which adopts the Los Angeles County Fire Code by reference which Christianson
hasn't been updated since 1871. The new amendments within the 2008 Fire Code require more restrictive Comments
building standards within the Very High Fire Severity Zones and will protect homes in Azusa requiring
sprinklers, fuel modifications, etc.
Mr. Art Morales addressed the Public Hearing reiterating the need for high standards in the foothill areas. A. Morales
Fire Battalion Chief Bob addressed the Public Hearing noting the importance of complying with the Fire Battalion
standards, especially those that five within High Fire Severity zones, of which the City adopted maps in Chief Comments
June of 2008.

SUBJECT: ACCEPTANCE OF IMPROVEMENTS FOR TRACT 63336-01 - MAPLETON
RECOMMENDATION:
It is recommended that the City Council: 1) accept the improvements required for Tract No. 63336-01 as
complete, 2) take the water, sewer and storm drain systems into the City maintained systems, 3) find that the
private streets have been completed, 4) accept the monumentation of the Tract as complete, and 5) transfer
the improvement bonds to a warranty bond and release the monument bond.
BACKGROUND:
On October 22, 2006, Tract No. 63336-01 was presented to the City Council by the subdivider PLC
Mapleton, LLC for approval, along with Improvement Securities in the amount of $421,648 and an
agreement that established the terms under which the City would allow the final map to record prior to the
completion of the tract improvements. On October 15, 2007, the City Council granted a reduction of the
improvement bonds to $42,164. The subdivider has now completed all required improvements and staff has
inspected the improvements to ensure that the City standards have been met. See the attached statements
from Light and Water Department and Quantum Consulting, the City's engineering inspection service.
With the recently completed monumentation of the street centerlines the bonds can now be released.
In accordance with the terms of the Subdivision agreement with the City of Azusa, specifically section 12 of
said agreement, all improvements approved by this action are warranted against "any material, defective
work or labor done, or defective materials furnished in the performance of this Agreement, including the
maintenance of all landscaping included in the Public Improvements in a vigorous and thriving condition
reasonably acceptable to City". The warranty period will end on December 1, 2009 if the City Council takes
action to approve this request tonight.

FISCAL IMPACT:
The Public Improvements will now be come the responsibility of the City, except for the maintenance
warranty provided for until December 1, 2009. No fiscal impacts are anticipated, as the cost of maintenance
was addressed in the Fiscal Impact Study approved as part of the Monrovia Nursery Specific Plan.

RECOMMENDATION
It is recommended that the City Council adopt the attached resolution approving Final Parcel
Map No. 63746.
BACKGROUND
The Planning Commission approved a tentative parcel map on October 10, 2007, to subdivide an
existing 15,038 square foot residential lot into two residential parcels with a single home
constructed on each lot located at 547 E. Tenth Street. The owner has submitted a final map for
this project and is seeking the Council's approval of this map.
All conditions of approval have been completed to the satisfaction of the City Engineer. The
final parcel map has been reviewed by Boghossian and Associates for accuracy and reviewed by
staff for compliance with the State Subdivision Map Act, City ordinances and is in substantial
compliance with the approved tentative parcel map.
FISCAL IMPACT
There are no fiscal impacts associated with this approval.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF AZUSA
DECLARING ITS APPROVAL OF FINAL PARCEL MAP NO. 63746
WHEREAS, the Azusa Planning Commission approved the tentative map of
Parcel No. 63746 on October 10, 2007; and
WHEREAS, Parcel Map No. 63746 has been properly prepared and signed.
NOW, THEREFORE, THE CITY COUNCIL OF THE CITY OF AZUSA
DOES HEREBY RESOLVE AS FOLLOWS:
Section 1. The City Engineer is hereby directed to approve Parcel Map
No. 63746. This approval is in conformance with the City of Azusa General Plan, City of
Azusa Municipal Code and conforms to the Subdivision Map Act.
Section 2. The City Clerk shall attest to this Resolution by signing the Title Sheet

RECOMMENDATION
It is recommended that the Redevelopment Agency Board of the City of Azusa ("Agency") approve the
proposed Amendment to the Five -Year Implementation Plan 2005-2009 ("Implementation Plan") for the
Merged Central Business District and West End Redevelopment Project Area ("Project Area"), which adds
streetscape improvement projects.
EXECUTIVE SUMMARY
Approval of the recommended actions will result in the addition of streetscape improvement projects to the
Implementation Plan.
BACKGROUND
California Redevelopment Law requires that Redevelopment Agencies prepare implementation plans every five
years to identify specific programs, which will eliminate blighting conditions throughout a project area. The
Agency adopted its most recent implementation plan on March 6, 2006, and conducted a Midterm review of the
Implementation Plan on June 16, 2008.
Agency staff is now proposing the following amendment to the Implementation Plan to further reduce and
eliminate blighting conditions throughout the Project Area.

The Proposed Amendment
The proposed amendment adds streetscape improvement projects to continue the redevelopment and
revitalization efforts currently underway. Agency staff proposes to install streetlights, landscape, trash
receptacles, benches, directional signage, and monumental signs to preserve the historic nature and character of
Downtown. A similar effort is proposed for the Foothill -Dalton area, with streetscape improvements matching
the Civic Center character.
The outlined streetscape improvement projects are intended to meet three of the Implementation Plan's
objectives: (1) Develop long-range plans to promote compatible land uses, establish design standards, and
construct essential public improvements; (2) develop Agency owned sites and assemble other key sites, and (3)
improve the business climate in the Central Business District and increase occupancy rates, rents and property
values.
FISCAL IMPACT
There is no fiscal impact associated with this proposed action. A detailed project budget will be outlined and
presented to the Agency for approval once Agency staff is ready to initiate the proposed projects.

Transmitted herewith is the Agency Treasurer's Statement of Cash Balances for the Redevelopment
Agency of the City of Azusa ("Agency") for the month of October 2008. Agency investments are
made in accordance with the Redevelopment Agency Investment Policy approved and adopted with
Resolution No. 08-R41 dated October 6, 2008, and Government Code Section 53601. Investment
activity is summarized in the "Treasury Book Balances -Cash and Investments" schedule, attached
herewith and an integral part of this report.
In October, Agency cash and investment balances decreased by $4,284,521.68. Cash received during
the month totaled $115,478.32, and total disbursements of $4,400,000.00 were made.
The Agency is the issuer of several Merged Project Tax Allocation Bonds. Of the total cash and
investments of $5,558,186.57, Wells Fargo Bank, the fiscal agent, held $2,371,091.76 on behalf of
the Agency. These funds are restricted for payment of debt service on the bonds and special bond -
funded projects. The remaining $3,187,094.81 was available for Agency operating, debt service,
restricted expenses, and outstanding liabilities.
FISCAL IMPACT
The balance of cash and investments and projected revenues for the next six months is expected to
be sufficient to meet cash disbursement requirements for at least the next six months.

The Board of Directors of the Redevelopment Agency of the City of Azusa met in regular session at the
above date and time in the Azusa Auditorium, 213 E. Foothill Blvd., Azusa CA.

TO DISCUSS THE FOLLOWING:
CONSIDERATION OF TWO FURNITURE, FIXTURE & EQUIPMENT LOANS: NANA'S FF&E Loans,
CREAMERY AND BAMBINO'S PIZZERIA; AND TWO ECONOMIC DEVELOPMENT PROGRAM EDLPLoans,
LOANS: BAMBINO'S PIZZERIA AND CANYON CITY BBQ. CONSIDERATION OF ECONOMIC & modify EDLP
DEVELOPMENT PROGRAM GUIDELINE MODIFICATIONS. Guidelines
Director of Economic and Community Development Christianson presented requests for Furniture, Fixture Dir Econ &Com
and Equipment Loan ("FF&E), Economic Development Program Loan ("EDLP"), and modification to the Dev Comments
employment requirement for EDLP participants from a three year period to a one year period. He advised
that Nana's Creamery has withdrawn request for loan. He detailed as follows: Loan request from
Bambino's Pizzeria for FF&E and EDLP, total $70,000; Loan request from Canyon City BBQ for EDLP
in the amount of $35,000; the request to modify the EDLP program to allow monitoring for one year,
instead of three. He responded to questions posed by Councilmembers/Drectors.
Moved by Director Macias, seconded by Director Gonzales and unanimously carried to approve a (1) a Approve FF&E
$28,000 Furniture, Fixture and Equipment Loan ("FF&E") for Nana's Creamery ("Nana's"), and (2) a Loans
$35,000 FF&E loan for Bambino's Pizzeria ("Bambino's), subject to Bambino's securing a direct lease
with the property owners.
Moved by Councilmember Macias, seconded by Councilmember Gonzales and unanimously carred to Approve EDLP
approve (1) a,$35,000 Economic Development Program Loan ("EDLP") for Bambino's, subject to Loans
Bambino's securing a direct lease with the property owners, (2) a $35,000 EDLP loan for Canyon City
BBQ ("Canyon BBQ"), and 3) modifications to the employment requirement for EDLP participants from a
three year period to a one year period.
Moved by Councilmember/Director Gonzales, seconded by Councilmember/Director Carrillo and Authorize CM
unanimously carried authorize the City Manager/Executive Director to execute all required FF&E and to execute docs
EDLP loan documents.
