NOTICE TO THE PUBLIC FOR REGULAR MEETING
In compliance with Government code Section 54957.5, copies of staff reports or other written documentation
relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E.
Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police
Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who
wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a
card requesting to speak and shall submit it to the City Clerkprior to 7:30 P.M.; cards submitted after will not
be accepted.

B. PUBLIC PARTICIPATION
This time has been set aside,for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this men
as an item other than a public headug item. Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker
has completed his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. ADOPTION OF GOALS AND POLICIES FOR THE IMPLEMENTATION OF THE MELLO-
ROOS COMMUNITY FACILITIES ACT OF 1982, AS_ AMENDED, CALIFORNIA
GOVERNMENT CODE ,� 53311, ET SEQ., REGARDING THE FORMATION OF COMMUNITY
FACILITIES DISTRICTS.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C30 establishing Goals and Policies for implementation of the Mello -Roos
Community Facilities Act of 1982, as amended. A RESOLUTION OF THE CITY COUNCIL OF THE
CITY OF AZUSA ADOPTING GOALS AND POLICIES FOR THE IMPLEMENTATION OF THE
MELLO-ROOS COMMUNITY FACILITIES ACT OF 1982, AS AMENDED, CALIFORNIA
GOVERNMENT CODE § 53311, ET SEQ., REGARDING THE FORMATION OF COMMUNITY
FACILITIES DISTRICTS.
2. ADOPT RESOLUTION OF INTENTION TO ESTABLISH CITY OF AZUSA COMMUNITY
FACILITIES DISTRICT NO. 2017-1 (PUBLIC „SAFETY SERVICES).
RECOMMENDED ACTION:
Accept the Certificate of the City regarding its representations of ownership of land proposed to be included
in the CFD; and Adopt Resolution No. 2017-C31 of Intention to establish the proposed CFD to partially
mitigate the costs of providing public safety services to new development located within the City. A
RESOLUTION OF INTENTION OF THE CITY COUNCIL OF THE CITY OF AZUSA WITH RESPECT
TO FORMATION OF PROPOSED CITY OF AZUSA COMMUNITY FACILITIES DISTRICT NO.
2017-1 (PUBLIC SAFETY SERVICES).

The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.
1. MINUTES OF THE REGULAR MEETING OF MAY 1 2017.
RECOMMENDED ACTION:
Approve minutes as written.
2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED APRIL 27,
2017 THROUGH MAY 10. 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C32 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.

3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
Section 5.7.5 and applicable Memorandum of Understanding(s).
4. AWARD A CONTRACT FOR THE SIERRA MADRE AVENUE STREET REHABILITATION
PROJECT (BETWEEN TODD AVENUE AND AZUSA AVENUE)— CIP PROJECT 66117H.
RECOMMENDED ACTION:
Award a contract for a total not -to -exceed amount of $844,470, which includes a 10% contingency for
potential change orders as needed to All American Asphalt for the Sierra Madre Avenue Street
Rehabilitation Project; and Authorize the City Manager to execute a contract, in a form acceptable to the
City Attorney, on behalf of the City.
5. APPROVAL OF FINAL TRACT MAP NO. 74154 FOR A 112 -UNIT TOWNHOME AND 1 -
COMMERCIAL BUILDING PROJECT LOCATED BETWEEN PALM DRIVE AND CITRUS
AVENUE IN THE TRANSIT II NEIGHBORHOOD OF THE ROSEDALE COMMUNITY.
RECOMMENDED ACTION:
Find that this project complies with the General Plan and is consistent with the approved tentative tract map
and any amendments thereto; and Find that this project is in compliance with the provisions of the
Subdivision Map Act; and Approve Final Tract Map 74154; and Authorize the City Clerk and City
Treasurer to endorse the certificates on the face of the map, which embodies the approval of Final Tract
Map 74154; and Authorize the City Manager to execute two Agreements for Completion of Public
Improvements for Tract 74154, Lots 1-6 and Lot 7.
6. CONSIDERATION OF RESOLUTION NO. 2017-C33 SETTING THE NUMBER OF
TRANSITIONAL FIREWORKS SALES PERMITS AVAILABLE FOR 2017.
RECOMMENDED ACTION:
Approve Resolution No.2017-C33 setting the number of transitional fireworks sales permits available for
2017.
7. APPROVE PURCHASE OF THIRTY SEVEN 37 CONDUCTED ENERGY DEVICES (TASER).
RECOMMENDED ACTION:
Waive the competitive bidding process due to sole source as authorized under Azusa Municipal Code
Section 2-520(b); and Approve the purchase of thirty seven CED's from Axon Enterprise, Inc. through the
TASER 60 purchase plan in an amount not to exceed $49,602.15; and Authorize the City Manager to
execute the TASER 60 purchase plan, in a form acceptable to the City Attorney, on behalf of the City.

8. APPROVE PURCHASE OF SIXTY-FIVE 65 REPLACEMENT POLICE BALLISTIC
HELMETS_
RECOMMENDED ACTION:
Waive the competitive bidding process as authorized under Azusa Municipal Code Section 2-521 (a) under
the General Services Administration (GSA) provision; and Authorize the purchase of ballistic helmets for a
not to exceed price of $29,027.49; and Authorize the City Manager to execute a Purchase Agreement with
Aardvark Tactical, in a form acceptable to the City Attorney, on behalf of the City.
9. CITY TREASURER'S STATEMENT OF CASH AND INVESTMENT BALANCES FOR THE
MONTH OF MARCH 2017.
RECOMMENDED ACTION:
Receive, review, and file the City Treasurer's Report for the month of March 2017.
10. AN APPLICATION TO CONDUCT THE AMERCIAN DIABETES ASSOCIATION TOUR DE
CURE BIKE EVENT THROUGH THE CITY OF AZUSA .
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C34, approving a special event permit for the Tour de Cure — American
Diabetes Association Bicycle Event.
11. CONTRACT AGREEMENTS EXECUTED BY CITY MANAGER FOR JANUARY THROUGH
APRIL OF 2017.
RECOMMENDED ACTION:
Receive and file the list of agreements executed by the City Manager for the months of January through
April 2017.
12. AWARD ANNUAL SCHOLARSHIPS.
RECOMMENDED ACTION:
Accept the recommendations of the Scholarship Selection Ad -Hoc Committee and award one (1) $1,000
scholarship; two (2) $500 scholarships; award three (3) $500 scholarships to the remaining three applicants
from the unused Adult Advancement Scholarship Opportunity; and Consider the Scholarship Selection Ad -
Hoc Committee's proposed modification to the Scholarship Program and give direction to Staff.
13. 1111RD QUARTER FINANCIAL REPORT ENDING MAfi('II 3I, 2017.
RECOMMENDED ACTION:
Receive, review, and file the third quarter financial report ending March 31, 2017.

F. SUCCESSOR AGENCY RELATED BUSINESS.
1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY
DATED APRIL 27, 2017 THROUGH MAY 10, 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-R07 allowing certain claims and demands to be paid out of Successor Agency
Funds.
