B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this a endo or are listed on this a ends
as an item other than a public hearing item. Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker
has completed his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. PUBLIC HEARING - FORMATION PROCEEDINGS FOR PROPOSED CITY OF AZUSA
COMMUNITY FACILITIES DISTRICT NO. 2017-1— RESOLUTIONS AND ORDINANCE.
RECOMMENDED ACTION:
1) Open the Public Hearing, receive public testimony, close the Public Hearing;
2) Adopt Resolution No. 2017-C40, Establishing City of Azusa Community Facilities District No. 2017-1
and Establishing the Boundaries Thereof.
3) Adopt Resolution No. 2017-C41, Calling a Special Election on Propositions with Respect to the Annual
Levy of Special Taxes and Establishing an Appropriations Limit.
If these resolutions are approved, the City Clerk will conduct the Special Election as described below and
tabulate the results. The City Clerk will then present a report to the City Council regarding the results, and if
the ballot propositions are approved, it is recommended that the City Council:
1) Adopt Resolution No. 2017-C42, Declaring the Results of the Consolidated Special Election within the
Community Facilities District.
2) Introduce, waive further reading, read by title only and adopt Ordinance No. 2017-04, Levying Special
Taxes to be Collected During Fiscal Year 2017-18 within the Community Facilities District.
2. PUBLIC HEARING - 2017 REFUSE RATE ADJUSTMENT PUBLIC HEARING.
RECOMMENDED ACTION:
1) Open the Public Hearing, receive public testimony, close the Public Hearing; and
2) Adopt Resolution No. 2017-C43, approving the annual refuse collection rate adjustment for Athens
Services effective July 1, 2017.
3. PUBLIC HEARING - FIRST READING OF ORDINANCE FOR A GENERAL PLAN
AMENDMENT GPA -2017-01) AND DEVELOPMENT CODE AMENDMENT (NO. 244) TO
REFLECT THE CITY'S ADOPTION OF THE TOD SPECIFIC PLAN. The Azusa TOD Specific Plan
Area encompasses 350 acres located in the central portion of the City of Azusa and is bounded by 9th Street
to the north, Angeleno Avenue to the west, 5th and 6th Streets to the south, and Citrus Avenue to the east.
No subsequent environmental review is necessary under the California Environmental Quality Act (CEQA)
for the proposed Specific Plan Amendment and General Plan Amendment based on Sections 15162 and
15164 of the CEQA Guidelines, California Code of regulations, Title 14, Chapter 3, as it can be seen with
certainty that: (1) there will be no new significant impacts or more severe significant impacts than were
previously disclosed in the EIR; (2) there have been no changes in the circumstances under which the
Specific Plan was approved and the EIR certified such that previously undisclosed impacts will occur; and
(3) no new information has come to light that was not known and could not have been known at the time the
EIR was certified showing that previously undisclosed impacts will occur.

RECOMMENDED ACTIO
1) Open the Public Hearing, receive public testimony, close the Public Hearing; and;
2) Adopt General Plan Amendment No. GPA -2017-01 and Code Amendment No. 244; and
3) Introduce, waive further reading and read by title only Ordinance No. 2017-05, Revising the General
Plan and Development Code to Reflect the Adoption of the TOD Specific Plan.
4. PUBLIC HEARING - FIRST READING OF ORDINANCE FOR A SPECIFIC PLAN
AMENDMENT (SPA -2017-01) AMENDING THE TRANSIT ORIENTED DEVELOPMENT
SPECIFIC PLAN. The Azusa TOD Specific Plan Area encompasses 350 acres located in the central
portion of the City of Azusa and is bounded by 9th Street to the north, Angeleno Avenue to the west, 5th and
6th Streets to the south, and Citrus Avenue to the east. No subsequent environmental review is necessary

5. PROPOSED BUSINESS LICENSE TAX AMNESTY PROGRAM.
RECOMMENDED ACTION:
1) Adopt Resolution No. 2017-C44, authorizing an amnesty program that would facilitate voluntary
compliance for businesses that have failed to obtain a business license tax, or underpaid their tax
obligation while waiving the payment of penalties or interest.
INTERIM URGENCY ORDINANCE OF THE CITY COUNCIL OF THE CITY OF AZUSA
CALIFORNIA ENACTED PURSUANT TO CALIFORNIA GOVERNMENT CODE SECTION
65858 TEMPORARILY PROHIBITING THE ESTABLISHMENT OF HOOKAH LOUNGES AND
TOBACCO STORES PENDING STUDY AND ADOPTION OF REGULATORY AND ZONING
STANDARDS.
RECOMMENDED ACTION:

1) Introduce, waive further reading, read by title only and adopt Urgency Ordinance No. 2017-07, enacted
pursuant to California Government Code Section 65858 Temporarily Prohibiting the Establishment of
Businesses Hookah Lounges and Tobacco Stores Pending Study and Adoption of Regulatory and
Zoning Standards.
7. FIRST READING OF ORDINANCE AMENDING SECTION 2-473 OF THE AZUSA MUNICIPAL
CODE RELATING TO THE PAYMENT OF LANDFILL FEES.
RECOMMENDED ACTION:
1) Introduce, waive further reading, and read by title only Ordinance No. 2017-08, amending Section 2-473
of the Azusa Municipal Code Relating to the Payment of Landfill Fees.
8. ADOPTION OF FISCAL YEAR 2017118 CITY UTILITIES AZUSA PUBLIC FINANCING
AUTHORITY AND SUCCESSOR AGENCY OPERATING AND CAPITAL IMPROVEMENT
PLAN BUDGETS.
RECOMMENDED ACTION:
1) Adopt Resolution No. 2017-C45, A Resolution of the City Council of the City of Azusa adopting the
budget and approving appropriations for the City of Azusa for the fiscal year commencing July 1, 2017
and ending June 30, 2018;
2) Adopt Resolution UB -07-2017, A Resolution of the Utility Board of the City of Azusa adopting the
budget and approving appropriations for Utilities for the fiscal year commencing July 1, 2017 and
ending June 30, 2018;
3) Adopt Resolution 2017-P1, A Resolution of the Azusa Public Financing Authority of the City of Azusa
adopting the budget and approving appropriations for the Authority for the fiscal year commencing July
1, 2017 and ending June 30, 2018; and
4) Adopt Resolution 2017-R08, A Resolution of the Successor Agency for the Former Redevelopment
Agency of the City of Azusa adopting the budget and approving appropriations for the Agency for the
fiscal year commencing July 1, 2017 and ending June 30, 2018.
E. CONSENT CALENDAR
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.

RECOMMENDED ACTION:
Approve minutes as written.

2. _RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED JUNE i,
2017 THROUGH JUNE 13, 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C46 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.
3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
Section 5.7.5 and applicable Memorandum of Understanding(s).
4. APPROVAL OF A CODE OF CONDUCT AND SERVICE/EMOTIONAL SUPPORT ANIMAL
POLICIES FOR AZUSA TRANSIT.
RECOMMENDED ACTION:
Approve the Code of Conduct/Transit Exclusion Policy; and approve the Service/Emotional Support Animal
Policy.
5. AUTHORIZATION TO ENTER INTO A FIRST STREET IMPROVEMENTS COOPERATIVE
AGREEMENT WITH THE CITY OF IRWINDALE TO PERFORM STREET IMPROVEMENTS
ON FIRST STREET FROM THE WESTERLY CITY BOUNDARY TO IRWINDALE AVENUE.
RECOMMENDED ACTION:
Approve the First Street Improvements Project Cooperative Agreement between the cities of Azusa and
Irwindale to perform street improvements on First Street from the westerly City boundary to Irwindale
Avenue; and authorize the City Manager to execute the First Street Improvements Project Cooperative
Agreement, subject to any non -substantive changes by the City Attorney, on behalf of the City.
6. APPROVAL OF BID AND AWARD OF CONTRACT TO THE CALIFORNIA SURFACING
CO. FOR ATHLETIC COURT RESURFACING PROJECT 441017A AT MEMORIAL
NORTHSIDE, SLAUSON AND GLADSTONE PARIS.
RECOMMENDED ACTION:
Approve bid and award a contract to The California Surfacing Co. in the amount of $59,750.00.
7. AUTHORIZATION TO RELEASE REQUEST FOR PROPOSALS FOR VIDEO PRODUCTION
SERVICES.
RECOMMENDED ACTION:
Authorize Staff to release the proposed Request for Proposals (RFP) for Video Production Services.

8. ADOPT RESOLUTION APPROVING AN ADMINISTRATIVE REGULATION RELATED TO
ELECTRONIC SIGNATURE USE POLICY REGULATING THE ACCEPTANCE OF
ELECTRONIC SIGNATURES AND ELECTRONIC OR ELECTRONICALLY SIGNED
RECORDS.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C47, approving an Administrative Regulation related to Electronic Signature
Use regulating the Acceptance of Electronic Signatures and Electronic or Electronically Signed Records.
9. APPROVAL OF BLANKET PURCHASE ORDERS OVER $25,000 FY 2017-18 (CITY &
SUCCESSOR AGENCY).
RECOMMENDED ACTION:
Approve the issuance of Blanket Purchase Orders detailed on the attached schedule, in an amount of
$25,000 or more as outlined in Sec. 2-518 (a) of the Azusa Municipal Code.
10. AUTHORIZATION TO RELEASE REQUEST FOR PROPOSALS FOR FIXED ASSET
INVENTORY SERVICES.
RECOMMENDED ACTION:
Authorize Staff to release the proposed Request for Proposals (RFP) for Fixed Asset Inventory Services.
11. APPROVE LICENSE AGREEMENT AND AMENDMENT TO GRANT RESOLUTION
RELATING TO USE OF THE AZUSA WOMAN'S CLUB FACILITY LOCATED AT 1003 N.
AZUSA AVENUE.
RECOMMENDED ACTION:
Approve the City's entry into a License Agreement allowing the Azusa Woman's Club to use the facility
for additional dates and times; and adopt Resolution No. 2017-C48, amending Resolution No. 6869
relating to the grant terms for the Azusa Woman's Club property to authorize the Azusa Woman's Club to
use the property for additional dates and times by lease, license or similar agreement with the City of
Azusa.
12. ADOPTION OF GANN APPROPRIATIONS LIMIT FOR FY 2017-18.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C49, approving and adopting the Annual Appropriations Limit for the Fiscal
Year 2017-18 in the Amount of $255,638,416.
13. APPROVAL OF CLASSIFICATION SPECIFICATION REVISIONS FOR LIBRARIAN
POSITION.
RECOMMENDED ACTION:
Approve the attached finalized revisions to the Librarian classification specification.

14. APPROVAL OF AGREEMENT BETWEEN AZUSA PACIFIC UNIVERSITY (APU) -AND THE
CITY OF AZUSA FOR UNIVERSITY RESOURCE OFFICER URO SERVICES.
RECOMMENDED ACTION:
Approve the attached one year agreement between the City of Azusa and Azusa Pacific University (APU) to
provide a full-time police officer to serve as a University Resource Officer (URO) for the 2017-2018 APU
undergraduate academic school year; and adopt Resolution No. 2017-050, authorizing Traffic Enforcement
on certain Private Streets within the City.
15. APPROVAL OF MEMORANDUM OF UNDERSTANDING AGREEMENT BETWEEN THE CITY
OF COVINA AND THE CITY OF AZUSA FOR JOINT TASKFORCE OPERATION
INVESTIGATIONS.
RECOMMENDED ACTION:
Approve a three (3) year Memorandum of Understanding (MOU) agreement between the City of Covina
and City of Azusa for joint taskforce operation investigations; and approve the City Manager to execute the
agreement, in a form approved by the City Attorney.
F. SUCCESSOR AGENCY RELATED BUSINESS.
1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY
DATED MAY 11 2017 THROUGH JUNE 13 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-R08 allowing certain claims and demands to be paid out of Successor Agency
Funds.
2. AUTHORIZATION TO COMMENCE PROCEEDINGS TO REFUND 2007A AND 2007B TAX
ALLOCATION BONDS.
RECOMMENDED ACTION:
Authorize Staff and the Agency's Financial Advisor to commence preparation of documents necessary for
the issuance of the Agency's (proposed) Tax Allocation Refunding Bonds, Series 2017.

1. SECOND READING AND ADOPTION OF ORDINANCE NO. 2017-02 AMENDING SECTION
88.42.035 MEDICAL MARIJUANA USES) OF CHAPTER 8$ DEVELOPMENT CODE OF
AZUSA CODE TO REGULATE THE PERSONAL MEDICAL. AND COMMERCIAL USE OF
MARIJUANA.
RECOMMENDED ACTION:
Read by title only, second reading and adoption, Ordinance No. 2017-02; amending Section 88.42.035
(Medical Marijuana Uses) of Chapter 88 (Development Code) of the Azusa Municipal Code to regulate the
personal, medical, and commercial Use of marijuana.

Parlors and Massage Technicians) to Article 4 (Standards for Specific Land Uses) of Chapter 88
(Development Code), Amending Section 88.70.020, Amending Section 88.24.005, and Amending Section
88.26.010 of the Azusa Municipal Code Regarding Massage Parlors and Massage Technicians.
