This time has been set aside for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this ageirrlet or are listed on this aizenda
as an item other than a public hearing item, Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to ,five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker

EXTENDING FOR A PERIOD OF TEN (10) MONTHS AND FIFTEEN (15) DAYS A
MORATORIUM TEMPORARILY PROHIBITING THE ESTABLISHMENT OF HOOKAH
LOUNGES AND TOBACCO STORES PENDING STUDY AND ADOPTION OF REGULATORY
AND ZONING STANDARDS.
RECOMMENDED ACTION:
1) Waive further reading, read by title only and adopt Urgency Ordinance No. 2017-09, an urgency
ordinance of the City Council of the City of Azusa, California: AN URGENCY ORDINANCE OF THE
CITY COUNCIL OF THE CITY OF AZUSA, IN ACCORDANCE WITH GOVERNMENT CODE
SECTION 65858, EXTENDING FOR A PERIOD OF TEN (10) MONTHS AND FIFTEEN (15) DAYS
A MORATORIUM TEMPORARILY PROHIBITING THE ESTABLISHMENT OF HOOKAH

RECOMMENDED ACTION:

(12) month extension of the Monrovia Nursery Development Agreement.
3. DESIGNATION OF VOTING DELEGATE AND ALTERNATE TO ATTEND THE LEAGUE OF

RECOMMENDED ACTION:
1) Appoint one (1) delegate to represent the City at the 2017 League of California Cities Annual

4. CONSIDERATION OF A RESOLUTION OF THE CITY OF AZUSA, CALIFORNIA, SETTING A
PUBLIC HEARING TO CONSIDER THE CONVEYANCE OF THAT CERTAIN REAL
PROPERTY LOCATED AT 17511 ARROW HIGHWAY, AZUSA, CALIFORNIA (ASSESSORS
PARCEL NO. 8621-024-905) TO CHARVAT FAMILY, LLC PURSUANT TO GOVERNMENT
CODE SECTION 37420 ET SEQ (PENDING SUCCESSOR AGENCY ITEM. —
RECOMMENDED ACTION:
1) Adopt Resolution No. 2017-052 making the required findings and setting the public hearing for August
21, 2017.
JOINT SUCCESSOR AGENCY AND CITY COUNCIL
CONSIDERATION OF A PURCHASE AND SALE AGREEMENT BETWEEN THE SUCCESSOR
AGENCY TO THE AZUSA REDEVELOPMENT AGENCY AND RAPHAEL GLENOAKS, LLC,
AND AN AGREEMENT AFFECTING REAL PROPERTY BETWEEN THE CITY OF AZUSA
AND RAPHAEL GLENOAKS, LLC FOR THE SALE AND DEVELOPMENT OF REAL
PROPERTY GENERALLY LOCATED 975 W. FOOTHILL BLVD. (APNS 8605-019-906,8605-019-
907).
RECOMMENDED ACTION:
Staff recommends that the Successor Agency take the following actions:
1) Approve the attached Agency Resolution No. 2017-R09 approving the Purchase and Sale Agreement.
2) Authorize the Executive Director of the Successor Agency to execute the Purchase and Sale Agreement.
Staff recommends the City Council take the following actions:
1) Approve the attached City Council Resolution No. 2017-053 approving the City Agreement.
2) Authorize the City Manager to execute the City Agreement.
6. APPROVAL OF QUITCLAIM DEEDS CONVEYING ASSESSOR PARCEL NO.'S 8684-019-900,
8608-028-901. 8608-028-903, 8608-028-904 8608-•028-908 8608-028-910,LAND 8621-024-905 FROM
THE SUCCESSOR AGENCY TO THE CITY OF AZUSA IN COMPLIANCE WITH THE CITY OF
AZUSA'S SUCCESSOR AGENCY LONG RANGE PROPERTY MANAGEMENT PLAN
RECOMMENDATIONS AND ACCEPTANCE BY THE CITY.
RECOMMENDED ACTION:
Approve and accept the Quitclaim Deeds conveying Assessor Parcel No.'s 8684-019-900, 8608-028-901,
8608-028-903, 8608-028-904, 8608-028-908, 8608-028-910, and 8621-024-905 from the Successor Agency
to the City of Azusa in compliance with the City of Azusa Successor Agency's long range property
management plan recommendations.

1. MINUTES OF THE REGULAR MEETINGS OF JUNE 19, 2017 AND SPECIAL MEETING OF

RECOMMENDED ACTION:
Approve minutes as written.
2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED JUNE 143
2017 THROUGH JULY 12, 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-054 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.
HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
Section 5.7.5 and applicable Memorandum of Understanding(s).
4. ADOPT RESOLUTION APPROVING THE AZUSA-RMC JOINT POWERS AUTHORITY
BUDGET FOR FISCAL YEAR 2017/2018.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-055 approving the Azusa -RMC Joint Powers Authority Budget for Fiscal Year
2017/2018.
5. REQUEST FOR PROPOSALS FOR AN ENVIRONMENTAL CONSULTANT FOR THE
PREPARATION OF ENVIROMENTAL IMPACT REPORT FOR THE SIERRA TODD PROJECT
LOCATED AT 1025 N. TODD AVENUE.
RECOMMENDED ACTION:
Authorize Staff to circulate a Request for Proposals (RFP) for environmental consultant services for the
preparation of the Environmental Impact Report (EIR) for the proposed Sierra Todd project, located at 1025
N. Todd Avenue.
6. APPROVE THE PLANS AND SPECIFICATIONS FOR THE FOOTHILL BOULEVARD AND
PALM DRIVE TRAFFIC SIGNAL PROJECT AND AUTHORIZE STAFF TO SOLICIT A
NOTICE OF INVITING BIDS — CIP PROJECT 66114A.

RECOMMENDED ACTION;
Approve the project plans and specifications; and authorize Staff to solicit a Notice of Inviting Bids for
Foothill Boulevard and Palm Drive Traffic Signal Project.
7. ACCEPTANCE OF PROJECT COMPLETION AND AUTHORIZATION TO FILE A NOTICE OF
COMPLETION FOR THE AZUSA AVENUE AND FIFTH STREET PARKING LOT
IMPROVEMENTS PROJECT NO. 73017D.
RECOMMENDED ACTION:
Accept the project improvements; and authorize Staff to file a Notice of Completion for the Azusa Avenue
and Fifth Street Parking Lot Improvements Project No. 73017D.
8. APPROVAL OF THE FIRST AMENDMENT TO THE MAINTENANCE SERVICES
AGREEMENT BETWEEN THE CITY OF AZUSA AND ULTIMATE MAINTENANCE
SERVICES, INC.
RECOMMENDED ACTION:
Approve the First Amendment to the Maintenance Services Agreement between the City and Ultimate
Maintenance Services, Inc. for city-wide janitorial services.
9. CONSIDERATION OF AZUSA CHAMBER OF COMMERCE CONTRACT FOR FISCAL YEAR
2017/2018.
RECOMMENDED ACTION:
Authorize Staff to finalize and execute the Agreement regarding Azusa Chamber of Commerce funding and
support between the City of Azusa and the Azusa Chamber of Commerce for Fiscal Year 2017/2018 at a
funding level of $44,493, which is the same funding level as Fiscal Year 2016/2017; and authorize the City
Manager to execute the contract, in a format acceptable to the City Attorney, on behalf of the City.
10. ADOPT RESOLUTION NO. 2017-056 APPROVING THE SUBMITTAL OF A GRANT
APPLICATION TO LOS ANGELES COUNTY METROPOLITAN TRANSPORTATION
AUTHORITY FOR THE TRANSIT ORIENTED DEVELOPMENT PLANNING GRANT
PROGRAM ROUND 5, IN THE TRANSIT ORIENTED COMMUNITIES_ _ TAX INCREMENT
FINANCING (TIF) PILOT PROGRAM, FOR A TIF_ FEASIBILITY_ STUDY.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-056, authorizing Staff to submit an application for grant funds for the Los
Angeles County Metropolitan Transportation Authority for the Transit Oriented Development Planning
Grant Program Round 5, in the Transit Oriented communities tax increment financing (TIF) Pilot Program,
for a TIF Feasibility Study; and approve funds in the amount of $7,900 to develop the full application only
if the City is selected to move forward in the application process.
11. APPROVAL OF A CONTRACT WITH ICE ENERGY HOLDINGS, INC. FOR THE
INSTALLATION OF AN ICE BEAR THERMAL ENERGY STORAGE SYSTEM AT CITY HALL.

RECOMMENDED ACTION:
Determine Ice Energy Holdings, Inc. to be a sole source vendor based on proprietary thermal storage
technology as authorized under Azusa Municipal Code Section 2-520(b); and approve a contract with Ice
Energy Holdings, Inc. to install an Ice Bear Thermal Energy Storage System and associated HVAC
equipment at City Hall; and approve funding for a not -to -exceed contract amount of $132,275.
12. APPROVAL OF THE NOTICE OF COMPLETION FOR THE C.I.P PROJECT 441017D AZUSA
CANYON BIKE TRAIL L.E.D. BOLLARD PROJECT.
RECOMMENDED ACTION:
Approval of the Notice of Completion for the C.I.P. Project #41017D Azusa Canyon Bike Trail L.E.D.
Bollard Project; and authorize Staff to file the Notice of Completion at the Los Angeles County Recorder's
office.
13. APPROVAL OF NEW CLASSIFICATION — PUBLIC WORKS MANAGER.
RECOMMENDED ACTION:
Approve the attached new class spec and recommended salary range for Public Works Manager.
14. CITY TREASURER'S STATEMENT OF CASH AND INVESTMENT BALANCES FOR THE
MONTH OF MAY 2017.
RECOMMENDED ACTION:
Receive, review, and file the City Treasurer's Report for the month of May 2017.
15. AZUSA UNIFIED SCHOOL DISTRICT AND AZUSA POLICE DEPARTMENT SCHOOL
RESOURCE OFFICER CONTRACT; JULY 1, 2017 - JUNE 30, 2018.
RECOMMENDED ACTION:
Approve the agreement for the 2017/18 school year between the Azusa Unified School District and the
Azusa Police Department, for the provision of law enforcement services at a shared cost; and authorize the
City Manager, or designee, to execute the agreement, in a form acceptable to the City Attorney, on behalf of
the City.
16. AUTHORIZATION TO ENTER INTO A MASTER AGREEMENT PROGRAM SUPPLEMENT
AGREEMENT, I+ [1ND EXCHANGE AGREEMENT, ANDIOR FUND TRANSFER AGREEMENT
WITH CALTRANS FOR THE ALOSTA AVENUE STREET IMPROVEMENTS PROJECT NO.
STPL-51 12(020).
RECOMMENDED ACTION:
Adopt Resolution No. 2017-057 and Authorize the Chief of Police/Acting City Manager to enter into a
Master Agreement, Program Supplement Agreement, Fund Exchange Agreement, and/or Fund Transfer
Agreement with Caltrans for the receipt of federal and/or state highway funds for the Alosta Avenue Street
Improvements Project No. STPL-5112(020).

17. REQUEST TO APPROVE PROFESSIONAL SERVICES AGREEMENT WITH MICHAEL
BAKER INTERNATIONAL TO PROVIDE ENVIRONMENTAL CONSULTANT SERVICES AND
MIG TO PROVIDE PLANNING CONSULTANT SERVICES FOR THE CALIFORNIA GRAND
VILLAGES AZUSA GREENS PROJECT LOCATED AT 919 W. SIERRA MADRE AVENUE.
RECOMMENDED ACTION:
Approve a Professional Services Agreement with Michael Baker International to provide environmental
consultant services in an amount not to exceed $175,405.00; and approve a Professional Services
Agreement with MIG to provide planning consultant services in an amount not to exceed $86,160.00; and
authorize the City Manager to execute the agreements, in a form acceptable to the City Attorney, on behalf
of the City.
18. ADOPT RESOLUTION NO. 2017-058 AUTHORIZING AND APPROVING POOLING OF SELF-
INSURANCE THROUGH THE EXCESS WORKERS' COMPENSATION PROGRAM OF THE
CALIFORNIA JOINT POWERS INSURANCE AUTHORITY.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-058, authorizing the transition from the first dollar workers' compensation
program to the $250K SIR based Workers' Compensation Program in the City's Membership with CJPIA;
and approve the necessary certifications to authorize and approve pooling of self-insurance through the
excess workers' compensation program of the California Joint Powers Insurance Authority, effective
August 1, 2017; and authorize and direct Staff to apply to the Department of Industrial Relations for a
Certificate of Consent to Self -insure, and to take such actions as necessary to effectuate self-insurance
worker's compensation for employees of the City.
F. SUCCESSOR AGENCY RELATED BUSINESS.
1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY
DATED JUNE 142 2017 THROUGH JULY 12, 2017;
RECOMMENDED ACTION:
Adopt Resolution No. 2017-R10 allowing certain claims and demands to be paid out of Successor Agency
Funds.
2. AUTHORIZATION TO ISSUE 2017 TAX ALLOCATION REFUNDING BONDS TO REFUND
2007A AND 2007B TAX ALLOCATION BONDS.
RECOMMENDED ACTI
Determine the preferred structure, Public Offering or Private Placement, for the refinancing of the Successor
Agency's outstanding Series 2007A and 2007B Tax Allocation Bonds; and Adopt Resolution No. 2017 -RI I
approve the issuance of Bonds in a not -to -exceed amount of $7.5 million in order to refund certain
outstanding bonds of the former Redevelopment Agency of the City of Azusa; and approve the execution
and delivery of an Indenture of Trust and Escrow Agreement, and provide for other matters properly
relating thereto.

RECOMMENDED ACTION:
Read by title only, second reading and adoption, Ordinance No. 2017-04, AN ORDINANCE LEVYING
SPECIAL TAXES TO BE COLLECTED DURING FISCAL YEAR 2017-2018 WITHIN THE
COMMUNITY FACILITIES DISTRICT
2. SECOND READING AND ADOPTION OF ORDINANCE NO. 2017-05 FOR A GENERAL PLAN
AMENDMENT GPA -2017-01) AND DEVELOPMENT CODE AMENDMENT (NO. 244) TO
REFLECT THE CITY'S ADOPTION OF THE TOD SPECIFIC PLAN.
RECOMMENDED ACTION:
Read by title only, second reading and adoption, Ordinance No. 2017-05, amending the General Plan and
Azusa's Development Code to reflect the City's adoption of the TOD Specific Plan.
3. SECOND READING AND ADOPTION OF ORDINANCE NO. 2017-06 FOR A SPECIFIC PLAN
AIVIENDMENT (SPA72017-01) AMENDING THE TRANSIT ORIENTED DEVELOPMENT
SPECIFIC PLAN.
RECOMMENDED ACTION:
Read by title only, second reading and adoption, Ordinance No. 2017-06, amending the Transit Oriented
Development Specific Plan.
4. SECOND READING AND ADOPTION OF ORDINANCE NO. 2017-08 AMENDING SECTION 2-
473 OF THE AZUSA MUNICIPAL CODE RELATING TO THE PAYMENT OF LANDFILL FEES.
RECOMMENDED ACTION:
Read by title only, second reading and adoption; Ordinance No. 2017-08 AMENDING SECTION 2-473 OF
THE AZUSA MUNICIPAL CODE RELATING TO THE PAYMENT OF LANDFILL FEES.
