SUBJECT: REQUEST FOR LEAVE OF ABSENCE WITHOUT PAY – WELDER/WATER
DISTRIBUTION WORKER I
SUMMARY:
An employee serving as Welder/Water Distribution Worker has been off work on approved family medical and
disability leave since November 16, 2016. Their 12 weeks of approved FMLA leave ended on February 6, 2017,
however they continue to be off work on a leave with pay using leave donations from City Staff until June 17,
2017. At this time, their leave time has been exhausted of all leave balances and they are requesting a leave of
absence without pay as provided for under Azusa Civil Service Rules, Section 6.8.2. This action authorizes a
leave of absence without pay.
RECOMMENDATION:
Staff recommends the City Council take the following action:
1) Approve an employee’s request for a leave of absence without pay beginning June 18, 2017 through
November 16, 2017, in accordance with Azusa Civil Service Rules, Section 6.8.1.
DISCUSSION:
A Welder/Water Distribution Worker has been off-work on extended leave. This employee has exhausted all
available leave balances and is requesting an unpaid leave of absence. In accordance with Azusa Civil Service
Rules, Section 6.8.1., the City Manager may grant a leave of absence without pay to any employee, upon
recommendation of the Department Head, for a period not to exceed five (5) days. The City Council, upon
recommendation of the City Manager, may grant a leave of absence without pay for a period not to exceed one
year.
Employee has submitted the appropriate written request with the Human Resources Department to continue leave
of absence without pay.

FISCAL IMPACT:
This extension would not result in any cost over what was budgeted for the 2017/2018 fiscal year budget.
