SUBJECT: ACCEPTANCE OF PROJECT COMPLETION AND AUTHORIZATION TO FILE A
NOTICE OF COMPLETION FOR THE SIDEWALK AND ADA IMPROVEMENTS
PROJECT NO. 601804-16
SUMMARY:
On June 5, 2017, the City Council awarded a contract to Martinez Concrete, Inc. for the Sidewalk and
Americans with Disabilities (ADA) Act Improvements Project No. 601804-16; funded by the City of
Azusa Community Development Block Grant (CDBG) Program (FY 2016-2017). The project was
completed on August 29, 2017. The proposed actions will accept the improvements and authorize Staff
to file a Notice of Completion for the project.
RECOMMENDATION:
Staff recommends that the City Council take the following actions:
1) Accept the project improvements; and
2) Authorize Staff to file a Notice of Completion for the Sidewalk and ADA Improvements Project
No. 601804-16.
DISCUSSION:
The completed project repaired uplifted/damaged sidewalks and curb ramps in compliance with the
latest standards for ADA requirements. The project focused on areas that qualified for CDBG funds. The
areas addressed were bounded by Sixth Street on the north, First Street on the south, Dalton Avenue on
the west, and Cerritos Avenue on the east.
The final improvements included forty-five (45) ADA curb ramps and 3,918 square feet of sidewalk.
Additional improvements were made to the curb ramp at the southwest corner of Foothill Boulevard and
Orange Avenue, within the CDBG approved area.

Although the project improvements were completed on August 29, 2017, the Community Development
Commission (CDC) of the County of Los Angeles needed to conduct their contract and labor
compliance review on October 31, 2017. The CDC approved the project, authorized release of the five
(5%) percent retention payment to the Contractor, and provided a Labor Compliance File Review
Clearance letter on November 1, 2017, pending Azusa City Council approval.
FISCAL IMPACT:
There is no fiscal impact associated with the recommended actions. The Sidewalk and ADA
Improvement Project No. 601804-16 was budgeted with $180,540 of CDBG funds, and it was
completed within budget at a total construction cost of $149,308.00.
