NOTICE TO THE PUBLIC FOR REGULAR MEETING
In compliance with Government code Section 54957.5, copies of staff reports or other written documentation
relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E.
Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police
Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who
wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a
card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not
be accepted.

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda
as an item other than a public hearing item. Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker
has completed his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. FIRST READING OF ORDINANCE REAUTHORIZING AND AMENDING ORDINANCE NO. 09-
O1 INCLUDING THE FEE PAID TO THE CITY BY STATE VIDEO FRANCHISE HOLDERS
FOR PUBLIC, EDUCATIONAL, AND GOVERNMENTAL ACCESS PURPOSES.
RECOMMENDED ACTION:
1) Waive further reading, read by title only and introduce the following Ordinance No. 2017-11:
AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF AZUSA REAUTHORIZING
ORDINANCE NO. 09-01 INCLUDING THE FEE PAID TO THE CITY BY STATE VIDEO
FRANCHISE HOLDERS FOR PUBLIC, EDUCATIONAL, AND GOVERNMENT ACCESS
PURPOSES
2. AUTHORIZATION OF TEMPORARY ASSISTANT CITY MANAGER POSITION TO ASSIST
WITH TRANSITION WITH NEW RECREATION AND FAMILY SERVICES DIRECTOR.
RECOMMENDED ACTION:
1) Authorize a temporary Assistant City Manager until December 31, 2017, to assist the Department of
Recreation and Family Services during the new Director transitional period.
E. CONSENT CALENDAR
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.
1. MINUTES OF THE REGULAR MEETING OF OCTOBER 2, 2017 AND SPECIAL MEETING OF
OCTOBER 9, 2017.
RECOMMENDED ACTION:
Approve minutes as written.
2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED
SEPTEMBER 28, 2017 THROUGH OCTOBER 11, 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2017-C67 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.

3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
Section 5.7.5 and applicable Memorandum of Understanding(s).
4. ACCEPTANCE OF THE NEWLY CONSTRUCTED AND GRANT DEED EXECUTION OF
CRAFTSMAN PARK ADJACENT TO TRACT 54057-2 LOCATED WITHIN THE NORTH PARK
NEIGHBORHOOD IN THE ROSEDALE COMMUNITY.
RECOMMENDED ACTION:
Accept the construction and grant deed of Craftsman Park located within the North Park Neighborhood and
between Sierra Madre Avenue and Los Angeles County Beatty Flood Control Basin; and Approve an
increase in the Landscape and Irrigation Contract with Brightview Landscape in the amount of $12,720.00
annually for the maintenance of these improvements; and Direct City staff to execute Grant Deed of
Craftsman Park.
5. APPROVE THE LEASE OF ONE LIFT-GATE TRUCK FROM ENTERPRISE FLEET
MANAGEMENT.
RECOMMENDED ACTION:
Waive competitive bidding as authorized under Azusa Municipal Code Section 2.521(a) and approve a lease
contract with Enterprise Fleet Management for the lease of one (1) lift-gate truck through the use of a bid
with TIPS-USA; and Authorize the Interim City Manager to execute a lease agreement with Enterprise Fleet
Management, in a form acceptable to the City Attorney, on behalf of the City; and Approve the change of
funds for the F-350 Lift Gate Truck (FY1718 Approved Capital Outlay item of $63,700) from 100%
AQMD fund to 50% Sewer Fund and 50% Gas Tax Fund.
6. CONTRACT AGREEMENTS EXECUTED BY CITY MANAGER FOR AUGUST AND
SEPTEMBER OF 2017.
RECOMMENDED ACTION:
Receive and file the list of agreements executed by the City Manager for the months of August and
September 2017.
7. APPOINTMENTS TO FILL VACANCIES ON CITY BOARD AND COMMISSIONS.
RECOMMENDED ACTION:
Adopt the following Resolution No. 2017-C68 re-appointing Dena Simpson to the Library Commission; and
Resolution No. 2017-C69 re-appointing Lita Abella to the Library Commission; and Resolution No. 2017-
C70 re-appointing Mitchell Loera to the Parks & Recreation Commission.

F. ADJOURNMENT.
The meeting will be adjourned in memory of Robert Wayne Lawver, Long Time Azusa Resident.
