NOTICE TO THE PUBLIC FOR REGULAR MEETING
In compliance with Government code Section 54957.5, copies of staff reports or other written documentation
relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E.
Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police
Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who
wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a
card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not
be accepted.

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda
as an item other than a public hearing item. Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker
has completed his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. APPROVE USE OF CITY HALL FRONT LAWN FOR AZUSA ROTARY WEEK LONG
VETERAN'S DAY CELEBRATION FLAG DISPLAY.
RECOMMENDED ACTION:
Approve the use of the front lawn at City Hall for the Azusa Rotary sponsored week long Veteran’s Day
Celebration Flag Display, “The Field of Glory.”
2. APPROVAL OF CONVEYANCE OF TITLE TO HILLSIDE LAND NORTH OF THE ROSEDALE
PROJECT FROM CITY TO AZUSA-RMC JOINT POWERS AUTHORITY.
RECOMMENDED ACTION:
Approve the City Manager’s Execution of Amended Grant Deed Conveying Hillside Property North of
Rosedale Project to the Azusa-RMC Joint Powers Authority.
E. CONSENT CALENDAR
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.
1. MINUTES OF THE REGULAR MEETING OF AUGUST 15, 2016.
RECOMMENDED ACTION:
Approve minutes as written.
2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY.
RECOMMENDED ACTION:
Adopt Resolution No. 2016-C55 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.
3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the Personnel Action Requests in accordance with the City of Azusa Civil Service Rules and
applicable Memorandum of Understanding(s).
4. APPROVAL OF EXTENSION OF THE CLINICAL AFFILIATION AGREEMENT BETWEEN
AZUSA PACIFIC UNIVERSITY SCHOOL OF NURSING AND THE AZUSA SENIOR CENTER.

RECOMMENDED ACTION:
Approve the extension of the Clinical Affiliation Agreement with Azusa Pacific University’s School of
Nursing for an additional three (3) years; and authorize the City Manager to execute, in a form acceptable to
the City Attorney, the agreement on behalf of the City.
5. APPROVE THE CITY’S RESPONSE TO THE 2015-16 CIVIL GRAND JURY REPORT
REGARDING APPOINTED COMMISSIONS.
RECOMMENDED ACTION:
Authorize the City Manager to sign and submit the City’s response letter regarding the report by the 2015 -
16 Civil Grand Jury regarding appointed commissions.
6. QUARTERLY LEGISLATIVE UPDATE.
RECOMMENDED ACTION:
Receive and file the Legislative Update.
7. APPROVE A MILITARY BANNER PROGRAM ESTABLISHING GUIDELINES AND
CRITERIA FOR THE PLACEMENT OF MILITARY BANNERS WITHIN THE CITY.
RECOMMENDED ACTION:
Approve the Military Banner Program establishing a set of guidelines and criteria for the placement of
Military Banners within the City.
8. ADOPT RESOLUTION DESIGNATING THE CITY TO SIGN CONTRACT AND RELATED
DOCUMENTS FOR THE ELDERLY NUTRITION PROGRAM AS REQUIRED BY THE LOS
ANGELES COUNTY AREA AGENCY ON AGING.
RECOMMENDED ACTION:
Accept the Los Angeles County Area Agency on Aging award of funding in the amount of $107,574 for the
next four years (2016 – 2020) for the Elderly Nutrition Program; and adopt Resolution No. 2016-C56,
designating the Mayor or his designee, City Manager, as the authorized representatives to sign the contract
and all documents necessary to accompany the contract, including all amendments to said contract within
fiscal years 2016-2020.
9. ACCEPT GRANT FROM CALIFORNIA OFFICE OF TRAFFIC SAFETY FOR THE
“SELECTIVE TRAFFIC ENFORCEMENT PROGRAM” (STEP) 2016-2017.
RECOMMENDED ACTION:
Approve Resolution No. 2016-C57 accepting the Grant agreement from the California Office of Traffic
Safety (OTS) awarding the City of Azusa up to $100,000.00 for hourly wages at an overtime rate to conduct
the outlined enforcement activity, purchase specified equipment, and attend approved training; and
authorize the City Manager to execute, in a form acceptable to the City Attorney, all documents necessary to
effectuate the receipt of the grant.

10. ADOPT RESOLUTION NO. 2016-C58 AUTHORIZONG CONTINUATION OF BENEFITS AND
SALARY FOR EMPLOYEES CALLED TO ACTIVE MILITARY DUTY UNDER TITLE 10 USC
ORDERS.
RECOMMENDED ACTION:
Adopt Resolution No. 2016-C58 Authorizing Continuation of Benefits and Salary for Employees Called to
Active Military Duty Under TITLE 10 USC Orders.
11. ADOPT RESOLUTION 2016-C59 APPROVING THE CITY OF AZUSA UNFUNDED LEAVE
BANK REDUCTION PROGRAM FOR AZUSA POLICE MANAGEMENT ASSOCIATION
(APMA) MEMBERS.
RECOMMENDED ACTION:
Adopt Resolution No. 2016-C59 of the City Council of the City of Azusa California approving the City of
Azusa Unfunded Leave Bank Reduction Program for Azusa Police Management Association (APMA)
Members.
12. FINDING OF PUBLIC BENEFIT TO THE COMMUNITY AT LARGE FOR RECOMMENDED
EXPENDITURES.
RECOMMENDED ACTION:
Approve expenditure in the amount of $200 to the Associated Students of Citrus College for the transportation of the
Gladstone High School Marching Band; and make a finding that the community at large will receive a public benefit
from the expenditure of funds for the recommended item.
13. ADOPTION OF RESOLUTIONS PERTAINING TO THE MARCH 7, 2017 GENERAL
MUNICIPAL ELECTION AND APPROVAL OF PROFESSIONAL SERVICES AGREEMENT
WITH MARTIN & CHAPMAN FOR ELECTION SERVICES AND SUPPLIES.
RECOMMENDED ACTION:
Adopt Resolution No. 2016-C60, calling for the holding of a General Municipal Election to be held on Tuesday,
March 7, 2017, for the election of certain officers as required by the provisions of the laws of the State of California
relating to General Law Cities; and adopt Resolution No. 2016-C61, requesting the Board of Supervisors of Los
Angeles County to render specified services to the City relating to the conduct of a General Municipal Election to be
held on Tuesday, March 7, 2017; and adopt Resolution No. 2016-C62, adopting regulations for candidates for elective
office pertaining to candidates statements submitted to the voters at the General Municipal Election to be held on
Tuesday, March 7, 2017; and approve the Professional Services Agreement with Martin & Chapman and authorize the
City Manager to execute the Agreement, in a form acceptable by the City Attorney, on behalf of the City for a total
compensation not to exceed $68,000.00 for election services and supplies.
14. ACCEPTANCE OF PROJECT COMPLETION AND AUTHORIZATION TO FILE A NOTICE OF
COMPLETION FOR THE ZACATECAS PARK PARKING LOT PROJECT NO. 66116C.
RECOMMENDED ACTION:
Accept the project improvements and authorize Staff to file a Notice of Completion for the Zacatecas Park
Parking Lot Project No. 66116C.

15. APPROVE RELEASE OF BONDS FOR TRACTS 54057-13 AND 54057-18 IN THE ROSEDALE
COMMUNITY.
RECOMMENDED ACTION:
Approve the release of the Performance Bond in the amount of $118,152.21, the Labor and Materials Bond
in the amount of $118,152.21, and the Monument Bond in the amount of $11,750.00 for Tract 54057-13;
and approve the release of the Performance Bond in the amount of $67,057.00, the Labor and Materials
Bond in the amount of $67,057.00, and the Monument Bond in the amount of $5,000.00 for Tract 54057-18.
16. CITY TREASURER’S STATEMENT OF CASH AND INVESTMENT BALANCES FOR THE
MONTH OF JULY 2016.
RECOMMENDED ACTION:
Receive, review, and file the City Treasurer’s Report for the month of July 2016.
17. AUTHORIZE NOTICE OF INTENT TO WITHDRAW FROM “LA WORKS” JOINTS POWERS
AUTHORITY.
RECOMMENDED ACTION:
Authorize the City Manager to Submit a Notice of Intent of the City to Withdraw from the “LA Works”
Joints Powers Authority.
18. REQUEST TO APPROVE PROFESSIONAL SERVICES AGREEMENT WITH LDM
ASSOCIATES, INC., FOR COMMUNITY DEVELOPMENT BLOCK GRANT ADMINISTRATIVE
PROFESSIONAL SERVICES.
RECOMMENDED ACTION:
Approve a Professional Services Agreement with LDM Associates, Inc. for Community Development Block Grant
(CDBG) administrative professional services in an amount not to exceed $42,000 for FY 2016-17; and authorize the
City Manager to execute the agreement, in a form acceptable to the City Attorney, on behalf of the City.
19. AWARD ANNUAL SCHOLARSHIP.
RECOMMENDED ACTION:
Accept the recommendations of the Scholarship Selection Ad Hoc Committee and award one (1) $1,000
scholarship; and two (2) $500 scholarships.
F. ORDINANCES/SPECIAL RESOLUTIONS
1. SECOND READING AND ADOPTION OF ORDINANCE NO. 2016-O6 – AMENDING SECTION
88.38.040(D) AND ADDING SECTION 88.38.085 TO THE AZUSA MUNICIPAL CODE TO
AUTHORIZE BILLBOARDS RELOCATION AGREEMENTS.
RECOMMENDED ACTION:
Conduct a second reading by title only, and adopt Ordinance No. 2016-O6, amending section 88.38.040(D)
and adding section 88.38.085 to the Azusa Municipal Code to authorize billboards relocation agreements.

2. SECOND READING AND ADOPTION OF ORDINANCE NO. 2016-O7 - AMENDING AND
RESTATING ARTICLE XVII OF THE AZUSA MUNICIPAL CODE REGARDING
REGISTRATION AND MAINTENANCE OF VACANT AND ABANDONED COMMERCIAL,
INDUSTRIAL PROPERTY AND OTHER RESIDENTIAL PROPERTIES.
RECOMMENDED ACTION:
Conduct a second reading, by title only, and adopt Ordinance No. 2016-O7, amending and restating Article
XVII – Registration and Maintenance of Vacant and Abandoned Commercial, Industrial and Other
Residential Properties.
