SUBJECT: REQUEST FOR LEAVE OF ABSENCE WITHOUT PAY – Police Officer
SUMMARY:
A Police Officer has been off work on approved family medical leave since December 23, 2015. At this time, the
Police Officer has exhausted all available leave balances and is requesting a leave of absence without pay as
provided for under Azusa Civil Service Rules. This action authorizes a leave of absence without pay.
RECOMMENDATION:
Staff recommends that the City Council take the following action:
1) Approve an employee’s request for a leave of absence without pay beginning February 13, 2016 through
March 25, 2016, in accordance with Azusa Civil Service Rules, Section 6.8.1.
DISCUSSION:
A Police Officer has been off-work on extended leave. This employee has exhausted all available leave balances
and is requesting an unpaid leave of absence. In accordance with Azusa Civil Service Rules, Section 6.8.1. the
City Manager may grant a leave of absence without pay to any employee, upon recommendation of the
Department Head, for a period not to exceed five (5) days. The City Council, upon recommendation of the City
Manager, may grant a leave of absence without pay for a period not to exceed one year.
Employee has submitted the appropriate written request with the Human Resources Department to continue leave
of absence without pay.

FISCAL IMPACT:
This extension would not result in any cost over what was budgeted for the 2015/2016 fiscal year budget.
