NOTICE TO THE PUBLIC FOR REGULAR MEETING In compliance with Government code Section 54957.5, copies of staff reports or other written documentation relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E. Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not be accepted. Presentation of Certificate to Kids Come First sponsors and Presentation of checks to Azusa and Gladstone High School Golf Teams. 2. Presentation of Certificate to Azusa Unified School District Nutrition Services. B. PUBLIC PARTICIPATION This time has been set aside for persons in the audience to make public comments on items within the subject matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda as an item other than a public hearing item. Members of the audience will have the opportunity to address the city council/agency board about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public comment has been received. Each person or representative of a group shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker has completed his/her comments. Public Participation will be limited to sixty (60) minutes. 10/1/2018 - 2 - C. REPORTS, UPDATES, AND ANNOUNCEMENTS FROM STAFF/COUNCIL 1. Request for a Proclamation proclaiming October 14-20, 2018 as "Freedom from Workplace Bullies Week" in the City of Azusa. 2. Request for a Proclamation proclaiming the Month of October 2018 as "National Community Planning Month". 3. Development Update — Staff will provide a brief update on various development projects in the City. (Oral Report) 4. Presentation of a video for Public Power Week. D. SCHEDULED ITEMS -None E. CONSENT CALENDAR The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered under SPECIAL CALL ITEMS. 1. MINUTES OF THE REGULAR MEETING OF SEPTEMBER 17, 2018 AND SPECIAL MEETING OF SEPTEMBER 25, 2018. RECOMMENDED ACTION: Approve minutes as written. 2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED SEPTEMBER 13, 2018 THROUGH SEPTEMBER 27, 2018. RECOMMENDED ACTION: Adopt Resolution No. 2018-054 allowing certain claims and demands and specifying the funds out of which the same are to be paid. 3. HUMAN RESOURCES ACTION ITEMS. RECOMMENDED ACTION: Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules Section 5.7.5 and applicable Memorandum of Understanding(s). 4. AUTHORIZATION TO ENTER INTO A MASTER AGREEMENT, PROGRAM SUPPLEMENT AGREEMENT, FUND EXCHANGE AGREEMENT, AND/OR FUND TRANSFER AGREEMENT WITH CALTRANS FOR THE CITYWIDE CRASH DATA ANALYSIS PROJECT NO. SSARPL- 5112 021 . 10/1/2018 -3 - RECOMMENDED ACTION: Approve Resolution No. 2018-055 and Authorize the City Manager to enter into a Master Agreement, Program Supplement Agreement, Fund Exchange Agreement, and/or Fund Transfer Agreement with Caltrans for the receipt of federal and/or state highway funds for the Citywide Crash Data Analysis Project No. SSARPL-5112(021). 5. REQUEST TO APPROVE AMENDMENT TO AGREEMENT WITH CALIFORNIA METRO PATROL FOR SECURITY SERVICES. RECOMMENDED ACTION: Approve the first amendment with California Metro Patrol an increased schedule, not -to -exceed annual amount of $89,500; and Authorize the City Manager to sign the first amendment between the City and California Metro Patrol; and Approve necessary budget amendments and adjust the current fiscal year budget accordingly. 6. CLASS SPECIFICATION REVISION AND TITLE CHANGE FROM BUYER TO PURCHASING AGENT. RECOMMENDED ACTION: Approve the revised classification specification and title change for the position of Buyer to Purchasing Agent. 7. CLASS SPECIFICATION REVISION AND TITLE CHANGE FROM UTILITY APPLICATION SPECIALIST TO UTILITY BUSINESS INFORMATION SPECIALIST. RECOMMENDED ACTION: Approve the revised classification specification and title change for the position of Utility Application Specialist to Utility Business Improvement Specialist. 8. CREATION OF NEW CLASSIFICATION — PLANNING AND ECONOMIC DEVELOPMENT MANAGER. RECOMMENDED ACTION: Approve the newly created classification for the position of Planning and Economic Development Manager. 9. REQUEST FOR EXTENSION OF WORKERS' COMPENSATION SALARY CONTINUATION — STREET MAINTENANCE CREW SUPERVISOR. RECOMMENDED ACTION: Grant the approval of an extension of leave for Street Maintenance Crew Supervisor, for the maximum period of six months per the SEIU MOU for a work related injury/illness. 10/l/2018 - 4 - 10. REQUEST FOR EXTENSION OF WORKERS' COMPENSATION SALARY CONTINUATION — PARK MAINTENACE WORKER III. RECOMMENDED ACTION: Grant the approval of an extension of leave for Park Maintenance Worker III, for the maximum period of six months per the ACEA MOU for a work related injury/illness. 11. AMENDMENT TO AGREEMENT FOR LIABILITY CLAIMS THIRD PARTY ADMINISTRATOR - CARL WARREN & COMPANY. RECOMMENDED ACTION: Approve amendment of the attached two-year service agreement between the City of Azusa and Carl Warren & Company. 12. FINDING OF PUBLIC BENEFIT TO THE COMMUNITY AT LARGE FOR RECOMMENDED EXPENDITURES TO THE ASSOCIATED STUDENTS OF CITRUS COLLEGE. RECOMMENDED ACTION: Approve an expenditure in the amount of $200 to the Associated Students of Citrus College for associated expenses with the 13`x' annual "Saluting Our Veterans"; and Make a finding that the community at large will receive a public benefit from the expenditure of funds for the recommended item. 13. APPOINTMENTS TO FILL VACANCIES ON CITY BOARD AND COMMISSIONS. RECOMMENDED ACTION: Adopt the following Resolutions: a) No. 2018-056 re -appointing Robert Donnelson to the Planning Commission. b) No. 2018-057 appointing Elizabeth Ramirez to the Planning Commission. c) No. 2018-058 re -appointing Edith Russek to the Cultural & Historical Preservation Commission. d) No. 2018-059 appointing Jenny Herrera to the Cultural & Historical Preservation Commission. e) No. 2018-C60 re -appointing Jennie Avila to the Library Commission. f) No. 2018-C61 re -appointing Fernando Rubio, Jr. to the Personnel Board. 14. COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) SIDEWALK AND ADA IMPROVEMENTS PROJECT NO. 601965-18. RECOMMENDED ACTION: Approve the proposed project; and Authorize staff to solicit a Notice of Inviting Bids for the CDBG-funded Sidewalk and ADA Improvements Project. 15. CONTRACT AGREEMENTS EXECUTED BY CITY MANAGER FOR JULY AND AUGUST 2018. RECOMMENDED ACTION: Receive and file the list of agreements executed by the City Manager for the months of July and August 2018. 10/1/2018 - 5 - 16. REQUEST FROM AZUSA CHAMBER OF COMMERCE TO WAIVE FEES FOR THE USE OF THE AZUSA AUDITORIUM. RECOMMENDED ACTION: Waive soft cost of rental fee associated with the use of the auditorium of $150 and recover hard costs for Custodian fee (Staff Time) of $80. F. SECOND READING ORDINANCES 1. SECOND READING OF ORDINANCE NO. 2018-04 PROHIBITING THE FEEDING OF WILD ANIMALS. RECOMMENDED ACTION: 1) Read by title only, second reading and adoption, Ordinance No. 2018-04: SECOND READING OF AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF AZUSA, CALIFORNIA ADDING SECTION 46-324 TO THE AZUSA MUNICIPAL CODE, RELATING TO PROHIBITIONS ON THE FEEDING OF WILD ANIMALS. G. SUCCESSOR AGENCY RELATED BUSINESS. 1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY DATED AUGUST 30, 2018 THROUGH SEPTEMBER 27.2018. RECOMMENDED ACTION: Adopt Resolution No. 2018-R15 allowing certain claims and demands to be paid out of Successor Agency Funds. H. ADJOURNMENT.
