PRESENT: Joe Torres, Pam Mercado,Gerard Batista and Rudy Garcia ROLL CALL STAFF: Joe Jacobs, Director of Recreation& Family Services Roy Chavez, Recreation Superintendent-Park Operations Marc Gonzales, Recreation Superintendent-Programs and Services Lenore Gonzales, Recreation Supervisor EXCUSED: Rossana Helbert Minutes of April 7, 2016,were approved by Gerard Batista seconded by Rudy Garcia. MINUTES APPROVED Consent calendar item#1-5, were approved with standard fees, conditions and/or modifications, on a CONSENT CALENDAR motion by Pam Mercado seconded by Gerard Batista. Request use of Memorial Park Gym, Monday to Thursday from June 6 to July 15, 2016 from DIVINE MPK GYM 6/6- 7:00 a.m.to 9:oo a.m. by Jeff Divine for volleyball practice. 7/15 2. Request use of Memorial Park Room 5 on December 17, 2016 from 11:00 a.m. to 4:00 p.m. by BAUTISTA MPK ROOM Jesus Bautista for christening reception. 512/17 3. Request use of tables, chairs and stage on November 12, 2016 from 5:00 p.m. to 11:45 p.m. by GONZALES Robert Gonzales for Azusa Chamber of Commerce event. EQUIPMENT 11/12 4. Request use of Memorial Park Gym on Tuesdays from June 7-July 12, 2016 from 9:3o a.m. to HASSEN MPK GYM 12:30 p.m. by Leila Hassen for Summer Reading Program. READING PROGRAM 5. Request use of Veteran's Park Gazebo on July 13,August 17 and December 17, 2016 from 5:oo MILLER VPK GAZEBO to 7:oo p.m. by Gina Miller for vaccination and licensing clinic for dogs and cats. 7/13, 8/17&12/17 Staff updated Commission on all programs and projects.
