Closed Session Report by the City Attorney B. PUBLIC PARTICIPATION This time has been set aside for persons in the audience to make public comments on items within the subject matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda as an item other than a public hearing item. Members of the audience will have the opportunity to address the city council/agency board about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public comment has been received. Each person or representative of a group shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker has completed his/her comments. CONSENT CALENDAR The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered under SPECIAL CALL ITEMS. 1. MINUTES OF THE REGULAR MEETING OF OCTOBER 15, 2018 AND SPECIAL MEETING OF OCTOBER 22, 2018. RECOMMENDED ACTION: Approve minutes as written. 2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED OCTOBER 10, 2018 THROUGH OCTOBER 31, 2018. RECOMMENDED ACTION: Adopt Resolution No. 2018-C64 allowing certain claims and demands and specifying the funds out of which the same are to be paid. 11/5/2018 - 3 - 3. HUMAN RESOURCES ACTION ITEMS. RECOMMENDED ACTION: Approve the Personnel Action Requests in accordance with the City of Azusa Civil Service Rules Section 5.7.5 and applicable Memorandum of Understanding(s). 4. APPROVE A MEMORANDUM OF UNDERSTANDING WITH FOOTHILL TRANSIT FOR THE RELOCATION OF THEIR SERVICE UTILITY BOX LOCATED ADJACENT TO THE FUTURE A-2 MIXED-USE DEVELOPMENT PROJECT AT AZUSA AVENUE AND NINTH STREET. RECOMMENDED ACTION: Authorize the City Manager to execute a Memorandum of Understanding (MOU) with Foothill Transit for the relocation of the Foothill Transit main switchgear on the Foothill Transit busway; and authorize the City Manager to execute, in a form acceptable to the City Attorney, on behalf of the City. 5. GRANT AGREEMENT FROM CA OFFICE OF TRAFFIC SAFETY FOR THE "SELECTIVE TRAFFIC ENFORCEMENT PROGRAM" (STEP) 2018-2019. GRANT NUMBER PT19005. RECOMMENDED ACTION: Approve Resolution No. 2018-C65 accepting a grant in the amount of $95,000.00 (for hourly wages at an overtime rate) from the California Office of Traffic Safety (OTS) to conduct DUI Saturation Patrols to apprehend drunk drivers, Warrant Service Operations and other related traffic programs; and authorize the Mayor to execute the grant agreement. 6. AWARD CONTRACT FOR THE COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) SIDEWALK AND ADA IMPROVEMENTS PROJECT NO. 601965-18 LOCATED WITHIN THE AREA BOUNDED BY FIFTH STREET ON THE NORTH, FOURTH STREET ON THE SOUTH, ORANGE AVENUE ON THE WEST, AND ALAMEDA AVENUE ON THE EAST. RECOMMENDED ACTION: Award a construction contract in the amount of $95,368, which includes 10% for contingency for potential change orders as needed, to Ruiz Concrete and Paving, Inc. for the Sidewalk and ADA Improvements Project No. 601965-18; and authorize the City Manager to execute a contract, in a form acceptable to the City Attorney, on behalf of the City. 7. CITY TREASURER'S STATEMENT OF CASH AND INVESTMENT BALANCES FOR THE MONTH OF AUGUST 2018. RECOMMENDED ACTION: Receive, review, and file the City Treasurer's Report for the month of August 2018. 11/5/2018 - 4 - 8. AWARD A CONTRACT FOR THE AZUSA LIBRARY AMERICANS WITH DISABILITIES ACT (ADA) COMPLIANT RESTROOM RETROFIT AND IMPROVEMENT PROJECT f COMMUNITY DEVELOPMENT BLOCK GRANT PROJECT (CDBG) 601863-171. RECOMMENDED ACTION: Award a contract for a total not -to -exceed amount of $250,555, which includes a 10% contingency for potential change orders as needed, to Corral Construction & Development, Inc., for the Azusa Library ADA Compliant Restroom Retrofit and Improvement Project (Community Development Block Grant Project 601863-17); and authorize the City Manager to execute a contract, in a form acceptable to the City Attorney, on behalf of the City. 9. RELEASE OF BONDS FOR TRACTS 54057-15 AND 54057-17 IN THE ROSEDALE COMMUNITY. RECOMMENDED ACTION: Release the Performance Bond in the amount of $578,535, the Labor and Materials Bond in the amount of $578,535 and the Monument Bond in the amount of $13,800 for Tract 54057-15; and release the Performance Bond in the amount of $363,529, the Labor and Materials Bond in the amount of $363,529, and the Monument Bond in the amount of $7,000 for Tract 54057-17. 10. APPROVE THE IMPROVEMENT PLANS FOR THE VERNON AVENUE FROM GLADSTONE STREET TO SANTA FE AVENUE PAVEMENT REHABILITATION PROJECT AND AUTHORIZE STAFF TO SOLICIT A NOTICE OF INVITING BIDS. RECOMMENDED ACTION: Approve the project plans; and authorize staff to solicit a Notice of Inviting Bids for the Vernon Avenue Pavement Rehabilitation Project. 11. CLASS SPECIFICATION REVISION — SENIOR PLANNER. RECOMMENDED ACTION: Approve the revised classification specification for the position of Senior Planner. 12. PURCHASE OF UNDERGROUND COPPER CABLE. RECOMMENDED ACTION: Approve the issuance of a purchase order to Wesco Distribution in an amount not to exceed $254,406. F. SUCCESSOR AGENCY RELATED BUSINESS. 1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY DATED SEPTEMBER 28, 2018 THROUGH OCTOBER 31, 2018. RECOMMENDED ACTION: Adopt Resolution No. 2018-R16 allowing certain claims and demands to be paid out of Successor Agency Funds. 11/5/2018 - 5 - G. ADJOURNMENT.
