SUBJECT: BFI GRANT TO THE AZUSA CITY LIBRARY
As a part of the City's continued effort to collaborate with local businesses in the City of Azusa,
staff is pleased to announce an exemplary collaboration between the City Library and BFI.
Through this effort, the City Library will be the recipient of$45,000 grant from BFI to
complement the Library's budget and provide additional and much needed resources and services
which will directly benefit the resident of our community.
This is the second time that City Library and BFI have cooperated and through this cooperation
additional services and programs have been provided for our residents.
It is our hope that this collaboration and partnership will open doors to other opportunities for
other partners and partnership in all areas of the City' services.
Staff at the Library are very pleased with this generous grant from BFI and we always explore
similar opportunities to provide additional services and programs for the community.

SUBJECT: PROCLAMATION FOR JOHN D. SANCHEZ AS THE CITY OF AZUSA'S
OFFICIAL SKYDIVER
Mr. John D. Sanchez is a permanent Maintenance Worker II in the Street Division. He has been
employed with the City of Azusa since March of 1991.
Mr. Sanchez was born in 1938 at the residential property of 810 N. Soldano Ave. He was raised
in a family of 19 members and to date, is still residing in Azusa. He has been married for 37
years now and has 8 children. He is the proud grandfather of 23 grandchildren.
Mr. Sanchez is a Korean Conflict Veteran of the Army's 14th Calvary and is actively involved in
the Veteran's of Foreign Wars, American Legion and Am-Vets. Mr. Sanchez is a Color Guard
volunteer, providing ceremonials for all occasions. His 85 skydiving jumps have qualified him
to become a member of the U.S. Parachute Association, Hispanic-American Airborne and the
Aztec Skydivers team. Mr. Sanchez is requesting Council approval to proclaim him as the City
of Azusa's official skydiver.
Upon approval, staff is requesting public acknowledgement with a proclamation at the Council
Meeting of November 6, 1995, under ceremonial. Upon approval, staff is requesting
authorization to design and fabricate a patch to be presented to Mr. Sanchez with the
proclamation.
RECOMMENDATION:
Staff is requesting Council approval to proclaim Mr. John D. Sanchez as the City of Azusa's
official Skydiver with a proclamation and fabrication of a patch to be presented by Council at the
November 6, 1995 City Council Meeting.

School students a certificate of recognition for being scholarship recipients by the Maintenance
Superintendents Association.
Please authorize the office of City Clerk to place this item under Ceremonial for the July 17, 1995

SUBJECT: REQUEST FOR CERTIFICATE OF APPRECIATION FOR SPONSORS
OF LIBRARY'S SUMMER READING PROGRAMS
BACKGROUND
For the past several years, children of the City of Azusa have had the opportunity to participate
in a series of educational and entertaining programs offered by the Azusa City Library during the
summer. During the course of these programs, the children of our community are provided with
an opportunity and exposure to live professional theater. It is through such experience that the
children's educational and cultural knowledge are enhanced.
Every year close to 1,000 youths participate in the Library's SUMMER READING
PROGRAMS and read close to 10,000 books during the 8 weeks of programs. Each
performance hosts 300 youngsters. Children receive a variety of incentives for their efforts in
reading during the summer months.
The funding for these programs has been provided entirely through the generous sponsorship of
local businesses and community groups. As a result of these successful programs, a permanent
partnership has been established between the library and the local businesses and community
groups and we have added more sponsors each year.
RECOMMENDATION
It is staff recommendation to award a certificate of appreciation to the following businesses and
groups for their continued support toward a very rewarding program for the youth of this
community. —

Subject: Police Helicopter Service Provided by the City of
Baldwin Park Police Department
In September 1994 Baldwin Park and Azusa entered into an
agreement for police helicopter services on an "as needed"
basis. The costs for this service were determined after each
time the helicopter was used for Azusa police needs. This
service proved to be very effective and efficient in certain
tactical situations. For example, the helicopter was able to
assist police ground units clear inaccessible canyon areas of
trespassers and bar-b-quers who posed fire safety hazards.
Also, the helicopter has proved extremely beneficial in
searching for suspects and for other crowd control
situations.
In July 1995, Baldwin Park proposed to enter into a one year
"flat rate" contract for the helicopter service. The flat
rate would provide unlimited helicopter responses on the
days/evenings when the helicopter is up in the air. The rate
is $700. 00 per month or $8, 400. 00 per fiscal year. We wanted
a one year trial on this contract to assess whether our
participation in the program is cost effective. The
helicopter primarily flies on weekends and holidays since
these are traditionally the busiest of times for law
enforcement.
For F/Y 95/96, $1, 000. 00 was budgeted for the "as needed"
helicopter service. The new proposal provided by Baldwin
Park is a very cost effective means in having helicopter
services available and will allow us the time necessary to
study the cost/benefits of the program. The remaining
$7,400. 00 will originate from the Asset-Seizure Account Funds
which are required to be used for police crime fighting
needs.
Three executed copies of the contract were forwarded to Azusa
after Baldwin Park' s City Council approved them at their
meeting on September 20, 1995. Each copy requires the
Mayor's signature. Two copies are to be returned to the City
Clerk in Baldwin Park. The Police Department will maintain
the files and contract on this service.

It is well known in law enforcement that the use of
helicopters is a tactical advantage for police officers. In
terms of officer safety, a helicopter is the fastest known
means to get an officer assistance or provide potential life
saving information from an aerial view. With our commitment
to providing the best service to the community that we can in
the most cost effective manner we can develop, I recommend
approval of this request for the remainder of the fiscal year
on a trial basis. If the determination is made that the
service is of benefit to our law enforcement efforts, then I
will seek to include it in the formal budget process.

AMENDMENT NO. 1 TO AGREEMENT FOR
HELICOPTER CONTRACT SERVICES
This Amendment No. 1 to the Agreement for Helicopter Contract
Services of September 7, 1994 is made and entered into this 1st day
of July, 1995 by and between the City of Baldwin Park, a Municipal
Corporation ( "Baldwin Park") , and the City of Azusa, a Municipal
Corporation ( "Azusa") .
RECITALS
WHEREAS, Baldwin Park and Azusa entered into a prior Agreement
on the seventh day of September, 1994, under which Baldwin Park
agreed to provide limited helicopter service to Azusa ( "Agree-
ment") ; and
WHEREAS, Baldwin Park and Azusa desire to amend this Agreement
with regard to the contract sum.
NOW, THEREFORE, it is hereby agreed that the Agreement for
Helicopter Contract Services dated September 7, 1994, between the
parties, is amended in the following particulars only:
1. Section 1 of the Agreement is hereby amended to read in
its entirety as follows:
Baldwin Park currently has available for
police services a Robinson R-22 Beta
Helicopter for approximately fifteen (15)
hours per week. Baldwin Park and Azusa wish
to have the helicopter available for limited
service in Azusa while the helicopter is on
site in Baldwin Park. Accordingly, Baldwin

Park shall provide limited helicopter service
to Azusa as follows:
SERVICE CHARGE
Unlimited responses $700 per month

RE: SELECTION OF CONSULTING ENGINEER TO ASSIST AZUSA TO DESIGN
TELECOMMUNICATION INFRASTRUCTURE
BACKGROUND
The Azusa City Council approved the Franchise Agreement with Charter Communications
in last council meeting. There are two provisions in the Agreement call for Charter to install
communication infrastructure for the city, namely I-NET and dark fiber. It is city's responsibility to
specify the number of fiber and the locations where the fiber will be terminated. City has 120 days
from the effective date of the Franchise Agreement to provide these information to Charter so that
Charter can incorporate city's requirements into its construction specifications.

FINDINGS
Staff has solicited Request for Qualifications from three consulting firms known to me to be involved
in telecommunications, has also interviewed the references these firms have furnished to me, and
received fee proposals based on the scope of services I outlined to them. Because of the undefined
nature of the system architect,the proposed fees ranged widely from$12,100 to $35,000. However,
the firm proposed $12,100 does exclude one of the services I specified but stated that for not more
than additional $18,000, it can accomplish all the tasks I specified.
In addition,I requested the consulting firm to provide periodic construction review and staff training
during Charter's construction period. The proposed costs for each trip range from $2,140 to $3,500.
RECOMMENDATION
Based on my review of each firm's qualifications, comments from references, and proposed fees, I
recommend the firm of Burns&McDonnell from Kansas City to assist Azusa to provide design and
specifications of the telecommunication infrastructure information to Charter Communications within

the schedule specified in the Charter's Franchise Agreement. Consulting fees for the initial phase
not to exceed $12,100, subsequent phase not to exceed $18,000, and each trip during the
construction will be at $2,140.
The Electrtic Division's Capital Improvement Project budget for this fiscal year includes $25,000 for
this project. I recommend that City Council approve an additional appropriation of$25,000 from
the Electric Retained Earnings to supplement the cost of this project.
