DELACH, CITY MANAGER //4j DATE: MAY 1 , 200%ti SUBJECT: APPROVAL OF PURCHASE ORDER WITH FISHER SCIENTIFIC COMPANY, LLC FOR THE PURCHASE OF MSA MILLENIUM GAS MASKS AND CBRN CANISTERS NOT TO EXCEED $17,012.20 FROM 2005 HOMELAND SECURITY GRANT RECOMMENDATION It is recommended that the City Council approve the selection of vendor, Fisher Scientific Company, LLC for the purchase of MSA Millennium Gas Masks and CBRN Canisters. Purchase not to exceed $17,012.20. BACKGROUND The Police Department recently requested bids for the purchase of NIOSH CBRN-approved MSA Millennium Gas Masks and CBRN Canisters to be used for protection against acts of Terrorism. Three bids were received in response: Fisher Scientific Company, LLC $17,012.20 Air Gas $22,009.99 Lab Safety $23,063.74 Staff is recommending that the purchase be awarded to Fisher Scientific Company, LLC as the lowest responsive bidder. DELACH, CITY MANAGERATAttO DATE: MAY 1, 2006 SUBJECT: REQUEST FOR LEAVE OF ABSENCE WITHOUT PAY- LIBRARIAN/YOUTH SERVICES RECOMMENDATION It is recommended that the City Council grant the request for a Leave of Absence without Pay from May 1, 2006 through May 12, 2006 in accordance with Azusa Civil Service Rules, Section 6.8, Subsection 6.8.1 Authorization. BACKGROUND In accordance with Azusa Civil Service Rules, Section 6.8, Leave of Absence without Pay, the City Council, upon recommendation of the City Manager, may grant a leave of absence without pay to any employee for a period not to exceed one year.
