A. PUBLIC PARTICIPATION
1. (Person/Group shall be allowed to speak without interruption up to five (5) minutes
maximum time, subject to compliance with applicable meeting rules. Questions to the speaker
or responses to the speaker’s questions or comments shall be handled after the speaker has
completed his/her comments. Public Participation will be limited to sixty (60) minutes time.)

The Consent Calendar adopting the printed recommended action will be enacted with one vote. If Staff

2. Acceptance of Grant of Permanent Easement for Water Utility Purposes from Property
Owner Rosedale Land Partners II LLC for Tract 54057-21 Portions of Lot C & E.
Recommendation: Accept the Grant of Permanent Easement by Resolution UB-1-2015 for
Water Utility purposes for Tract 54057-21 portions of Lot C and E in the City of Azusa and
authorize the City Clerk to file said easement documents with the Office of the Los Angeles
County Recorder.

Permanent Easement by Resolution UB-2-2015 for Water Utility purposes for the property,
Tacos El Gavilan, located at 887 S. Azusa Avenue in the City of Azusa and authorize the City
Clerk to file said easement documents with the Office of the Los Angeles County Recorder.

4. Approval of Notice of Completion for the Installation of Project W-276, Truck Storage
Shed, at the Gladstone Yard. Recommendation: Accept the Storage Shed Project W-276,
approve the Notice of Completion, and direct the City Clerk’s Office to execute the Notice of

5. Approval of Notice of Completion for Project W-281 Installation of Sludge Drying Bed
Improvements at Hsu Canyon Water Treatment Plant. Recommendation: Accept Project
W-281, approve the Notice of Completion and direct the City Clerk’s Office to execute the

6. Approve Lease of City Property at the Dalton Reservoir Site to T-Mobile for a Cell Phone
Communications Tower. Recommendation: Approve a lease to T-Mobile West Corporation at
a price of $2,600 per month for a portion of City Property at the Dalton Reservoir site for a cell
phone tower and authorize the City Manager to execute the lease agreement as approved by the
City Attorney.

7. Approval to Hire Stetson Engineers to Prepare Discharge Permit Applications to Test
Pump Aspan Well. Recommendation: Approve the selection of Stetson Engineers to prepare a
discharge application necessary to test pump Azusa Light & Water’s Aspan Well.

8. Waive Formal Bidding Procedures and Award Purchase of One (1) Ford F150 Pickup
Truck and One (1) Ford F350 Pickup Truck to Downtown Ford Sales Under California
Multiple Award Schedule (CMAS). Recommendation: Waive formal sealed bids in
accordance with AMC Section 2-522, sub-section C, and award the purchase of one (1) Ford
F150 pickup truck for $22,333.01 and one (1) Ford F-350 pickup truck for $38,313.96 to
Downtown Ford Sales under the California Multiple Award Schedule (CMAS Contract No. 1-
08-23-20) in the total amount of $60,646.97 including tax and delivery.

9. Approve Purchase of a Sleeve Valve for the Interconnection Line Between the SGVMWD
Water Pipeline and the Hsu-Canyon Water Treatment Plant Project W-280.
Recommendation: Approve the purchase a 14-inch Bailey sleeve valve for the future
supplemental water supply line to the Hsu-Canyon Water Treatment Plant and approve the
attached Resolution UB-3-2015 authorizing a budget amendment of $170,000 in the Water Fund.

10. Approval of San Juan Unit 3 Energy Exchange and Firming Arrangement Between Azusa
and Arizona Public Service Company (APS). Recommendation: Authorize Staff to renew the
energy exchange and firming arrangement for San Juan Unit 3 with Arizona Public Service
(APS) for the period of February 1, 2015 through December 31, 2015 and authorize the Director
of Utilities to execute the associated final agreement with APS.
