General public comment will be provided for during the regular portion of the meeting beginning at 7:30 P.M. 1. CONFERENCE WITH REAL PROPERTY NEGOTIATORS – Pursuant to California Government Code, Section 54956.8. NOTICE TO THE PUBLIC FOR REGULAR MEETING In compliance with Government code Section 54957.5, copies of staff reports or other written documentation relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E. Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not be accepted. PUBLIC PARTICIPATION This time has been set aside for persons in the audience to make public comments on items within the subject matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda as an item other than a public hearing item. Members of the audience will have the opportunity to address the city council/agency board about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public comment has been received. Each person or representative of a group shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker has completed his/her comments. Public Participation will be limited to sixty (60) minutes. 3/4/2019 - 3 - C. REPORTS, UPDATES, AND ANNOUNCEMENTS FROM STAFF/COUNCIL 1. Mayor Rocha: Request for Proclamation declaring April 2019 as “Donate Life Month” in the City of Azusa. D. SCHEDULED ITEMS 1. PUBLIC HEARING: CONSIDERATION OF CALIFORNIA GRAND VILLAGE AZUSA GREENS PROJECT ENTITLEMENTS: (1) CERTIFICATION OF THE CALIFORNIA GRAND VILLAGE ENVIRONMENTAL IMPACT REPORT (SCH. NO. 2018061063) AND MITIGATION MONITORING AND REPORTING PLAN; (2) SPECIFIC PLAN (SP-2017-02); (3) GENERAL PLAN AMENDMENT (GPA-2017-02); (4) ZONE CHANGE (Z-2017-01); (5) SENIOR HOUSING DESIGN REVIEW (DR-2017-20); (6) GOLF COURSE DESIGN REVIEW (DR-2018-04); AND (7) TENTATIVE TRACT MAP (TTM-81336) LOCATION: 1100 NORTH TODD AVENUE AZUSA (FORMERLY 919 WEST SIERRA MADRE AVENUE. CEQA: A Final Draft Environmental Impact Report for the project was released for public review on November 28, 2018 to January 28, 2019. The Final Environmental Impact Report (SCH No. 2018061063) was released on February 21, 2019. RECOMMENDED ACTION: 1) Receive a brief staff report, open the public hearing and consider any written/oral information received regarding the proposed Project; and 2) Provide direction to staff concerning any additional information to be researched and provided to the City Council to assist in evaluating the Project; and 3) Keep the public hearing open and continue the Public Hearing and the consideration of the Project to April 1, 2019 [or April 15, 2019]. E. CONSENT CALENDAR The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered under SPECIAL CALL ITEMS. 1. MINUTES OF THE REGULAR MEETING OF FEBRUARY 19, 2019. RECOMMENDED ACTION: Approve minutes as written. 2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED FEBRUARY 15, 2019 THROUGH FEBRUARY 27, 2019. RECOMMENDED ACTION: Adopt Resolution No. 2019-C07 allowing certain claims and demands and specifying the funds out of which the same are to be paid. 3/4/2019 - 4 - 3. HUMAN RESOURCES ACTION ITEMS. RECOMMENDED ACTION: Approve the Personnel Action Requests in accordance with the City of Azusa Civil Service Rules Section 5.7.5 and applicable Memorandum of Understanding(s). 4. ACCEPTANCE OF PROJECT COMPLETION AND AUTHORIZATION TO FILE A NOTICE OF COMPLETION FOR THE WEST WING CEILING ABATEMENT PROJECT LOCATION: 213 E. FOOTHILL BLVD. RECOMMENDED ACTION: Accept the project improvements; and Authorize Staff to file a Notice of Completion for the West Wing Ceiling Abatement Project. 5. AUTHORIZATION TO PURCHASE AND INSTALLATION OF (2) POOL HEATERS FOR THE JAMES SLAUSON AQUATICS CENTER LOCATION: 501 E. 5TH STREET. RECOMMENDED ACTION: Authorize the purchase and installation of (2) Raypak MVB heaters by Commercial Aquatics Services in the amount of $36,563.30. 6. AUTHORIZATION TO PURCHASE (3) HIGH RATE SAND FILTRATION FILTERS FOR THE JAMES SLAUSON AQUATICS CENTER LOCATION: 501 E. 5TH STREET. RECOMMENDED ACTION: Waive competitive bidding as authorized under Azusa Municipal Code Section 2.521(a) and approve the purchase and installation of (3) EPD, USA manufactured, high rate sand filtration filters to Knorr Systems, Inc. in the amount of $49,383.14. 7. REQUEST FOR PROPOSALS (RFP) FOR HISTORIC CONTEXT STATEMENT AND UPDATE CITYWIDE HISTORIC SURVEY. RECOMMENDED ACTION: Authorize staff to circulate a Request for Proposals (RFP) for a Historic Context Statement and Update Citywide Historic Survey. 8. AUTHORIZATION OF EXPENSE FROM CAPITAL RESERVE FUND FOR ITEMS RELATING TO LIBRARY SEWER SPILL LOCATION: 729 N. DALTON AVE. RECOMMENDED ACTION: Authorize a Capital Reserve Fund expenditure equal to $329,733.04 for the total amount of the project with a reimbursement of $173,746.00 from the CJPIA. 3/4/2019 - 5 - F. SUCCESSOR AGENCY RELATED BUSINESS. 1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY DATED JANUARY 31, 2019 THROUGH FEBRUARY 27, 2019. RECOMMENDED ACTION: Adopt Resolution No. 2019-R05 allowing certain claims and demands to be paid out of Successor Agency Funds. G. ADJOURNMENT.
