A. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items within the
subject matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on
this agenda as an item other than a public hearing item. Members of the audience will have the
opportunity to address the city council/agency board about public hearing items at the time the public
hearing is held. Under the provisions of the Brown Act, the council/agency board is prohibited from taking
action on oral requests, but may refer the matter to staff or to a subsequent meeting. The council/agency
board will respond after public comment has been received. Each person or representative of a group
shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance
with applicable meeting rules. Questions to the speaker or responses to the speaker's questions or
comments shall be handled after the speaker has completed his/her comments. Public Participation will be
limited to sixty (60) minutes.

C. SCHEDULED ITEMS
1. AUTHORIZATION TO ENTER INTO EMPLOYMENT AGREEMENT FOR CITY
MANAGER—TROY BUTZLAFF.
RECOMMENDED ACTION:
Approve the Employment Agreement for Troy Butzlaff as City Manager for the City of Azusa, subject to any
non-substantive changes made by the City Attorney and Interim City Manager.
2. PUBLIC HEARING — COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) FY
2015/16,ADOPTION OF RESOLUTION APPROVING THE 41ST YEAR CDBG PROGRAM.
RECOMMENDED ACTION:
Open the Public Hearing, receive testimony, close the Public Hearing and adopt Resolution No.
2015-C3, approving the 41st Year Community Development Block Grant(CDBG) Program.
3. PUBLIC HEARING - AN APPLICATION TO CONDUCT A MARATHON AND A HALF-
MARATHON FOOTRACE IN 2015 AND AGAIN IN 2016 (REVEL CANYON CITY 2015
MARATHON & REVEL CANYON CITY 2016 MARATHON) THROUGH THE CITY OF
AZUSA. APPLICANT: BROOKSEE, LLC. The project is exempt from California Environmental
Quality Act (CEQA) per Section 15061(b)(3) general rule that CEQA applies only to projects which
have the potential for causing a significant effect on the environment. The proposed project marathon
will be conducted on existing public streets during the day without the expectation of that it will have
a significant effect on the environment.
RECOMMENDED ACTION:
Open the Public Hearing, receive testimony, close the Public Hearing, and approve the application for
the REVEL Canyon City 2015 and 2016 Marathons.
D. CONSENT CALENDAR
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wishes to address any item on the Consent Calendar individually, it will be
considered under SPECIAL CALL ITEMS.

RECOMMENDED ACTION:
Approve minutes as written.

2. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE CITY.
RECOMMENDED ACTION:
Adopt Resolution No. 2015-C4 allowing certain claims and demands and specifying the funds out of
which the same are to be paid.
3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve Personnel Action Requests in accordance with the City of Azusa Civil Service Rules and
applicable Memorandum of Understanding(s).
4. APPROVAL OF JANITORIAL SERVICES SPECIFICATIONS AND AUTHORIZATION TO
SOLICIT BIDS.
RECOMMENDED ACTION:
Approve the Janitorial Services Specifications and authorize staff to solicit bids for the City-wide
Janitorial Services.
5. APPROVAL TO FULFILL LEASE AGREEMENT BETWEEN THE POLICE
DEPARTMENT AND AARDVARK TACTICAL,INC.ATHENA PROGRAM.
RECOMMENDED ACTION:
Authorize the Chief of Police to pay Aardvark Tactical Inc., ATHENA Program, located at 1935
Puddingstone Dr., La Verne, CA 91750 the sum of$23,944.06, in accordance with the Five (5) year
lease agreement approved by Council on July 18, 2011.
6. CITY TREASURER'S STATEMENT OF CASH AND INVESTMENT BALANCES FOR THE
MONTH OF DECEMBER 2014.
RECOMMENDED ACTION:
Receive, review and file the City Treasurer's Report for the month of December 2014.
7. REQUEST FOR LEAVE OF ABSENCE WITHOUT PAY — COMMUNITY SERVICE
OFFICER.
RECOMMENDED ACTION:
Grant the request for a Leave of Absence Without Pay beginning January 6, 2015 through February
28, 2015, in accordance with Azusa Civil Service Rules, Section 6.8, and Subsection 6.8.1
Authorization.

F. ORDINANCE/SPECIAL RESOLUTIONS
1. SECOND READING AND ADOPTION OF AN ORDINANCE OF A DEVELOPMENT CODE
AMENDMENT (ZCA-236), A CITY-INITIATED REQUEST TO AMEND VARIOUS
SECTIONS OF CHAPTER 88 OF THE "DEVELOPMENT CODE, MARCH 2005" (2005
CODE) OF THE CITY OF AZUSA MUNICIPAL CODE. The proposed Code Amendments are not
subject to the California Environmental Quality Act ("CEQA") pursuant to Sections 15060(c)(2)
because the activity, which involves clarifying and recodifying Sections of Chapter 88 of the City of
Azusa Municipal Code, will not result in a direct or reasonably foreseeable indirect physical change in
the environment; and 15060(c)(3) because the activity is not a project as defined in Section 15378 of the
CEQA Guidelines, California Code of regulations, Title 14, Chapter 3, because it has no potential for
resulting in physical change to the environment, directly or indirectly. (Public Hearing was held on
January 5, 2015).
RECOMMENDED ACTION:
Waive further reading, and adopt Ordinance No. 2015-01
2. SECOND READING AND ADOPTION OF AN ORDINANCE OF A DESIGN REVIEW (DR-
2014-31) AND WORLD GYM DEVELOPMENT AGREEMENT APPLICATION TO ALLOW A
NON-RETAIL FITNESS CENTER (WORLD GYM) TO OCCUPY APPROXIMATELY 35,000
SQUARE FEET OF THE EXISTING EDGEWOOD SHOPPING CENTER AND EXTERIOR
MODIFICATIONS ON THE SOUTH AND EAST FACADE LOCATED WITHIN THE CITY'S
DE (EDGEWOOD DISTRICT) ZONE. This project is categorically exempt from environmental
review per Section 15301 — Existing Facilities of CEQA (California Environmental Quality Act).
(Public Hearing was held on January 5, 2015).
RECOMMENDED ACTION:
Waive further reading, and adopt Ordinance No. 2015-02.

RECOMMENDATION:
It is recommended that the Council Members receive, review, and file the City Treasurer's
Report for the City of Azusa for the month of December 2014.
BACKGROUND:
Transmitted herewith is the City Treasurer's Statement of Cash Balances for the City of Azusa
for the month of December 2014. City investments are made in accordance with the City's
Investment Policy adopted and approved with Resolution No. 05-C16 dated, February 22, 2005,
and Government Code Section 53600 et seq.
FISCAL IMPACT:
The balances of cash, investments, and projected revenues for the next six months are expected
to be sufficient to meet cash disbursement requirements of the City for at least the next six
months.
The change in total cash in bank and investments (at cost) from November 30, 2014 to December
31, 2014 is a net increase of$1,761,193.83.
The change in total cash in bank and investments (at market) from November 30, 2014 to
December 31, 2014 is a net increase of$1,631,785.50.

RECOMMENDATION
It is recommended that the City Council grant the request for a Leave of Absence without Pay
beginning January 6, 2015 through February 28, 2015, in accordance with Azusa Civil Service
Rules, Section 6.8, and Subsection 6.8.1 Authorization.
BACKGROUND
In accordance with Azusa Civil Service Rules, Section 6.8 Leave of Absence without Pay,the City
Council, upon recommendation of the City Manager, may grant a leave of absence without pay to
any employee for a period not to exceed one year. (The City Manager may grant a leave of absence
for a period not to exceed five (5) days).
FISCAL IMPACT
There is no fiscal impact.
