NOTICE TO THE PUBLIC FOR REGULAR MEETING In compliance with Government code Section 54957.5, copies of staff reports or other written documentation relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E. Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.ci.azusa.ca.us. Persons who wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not be accepted. Presentation of Certificate of Recognition to the 2019 Community Awareness Artwork Contest winners, Blanca Garcia, Denisse Aranda, Aleyna Gomez and Angela Reina. 2. Presentation of Proclamation proclaiming April 18, 2019 as Arbor Day in the City of Azusa. 4/1/2019 - 3 - B. PUBLIC PARTICIPATION This time has been set aside for persons in the audience to make public comments on items within the subject matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda as an item other than a public hearing item. Members of the audience will have the opportunity to address the city council/agency board about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public comment has been received. Each person or representative of a group shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker has completed his/her comments. Public Participation will be limited to sixty (60) minutes. C. REPORTS, UPDATES, AND ANNOUNCEMENTS FROM STAFF/COUNCIL D. SCHEDULED ITEMS - None E. CONSENT CALENDAR The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered under SPECIAL CALL ITEMS. 1. MINUTES OF THE REGULAR MEETING OF MARCH 4, 2019 AND MARCH 18, 2019. RECOMMENDED ACTION: Approve minutes as written. 2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED MARCH 14, 2019 THROUGH MARCH 27, 2019. RECOMMENDED ACTION: Adopt Resolution No. 2019-C09 allowing certain claims and demands and specifying the funds out of which the same are to be paid. 3. HUMAN RESOURCES ACTION ITEMS. RECOMMENDED ACTION: Approve the Personnel Action Requests in accordance with the City of Azusa Civil Service Rules Section 5.7.5 and applicable Memorandum of Understanding(s). 4. APPROVAL OF A PARKING LICENSE AGREEMENT BETWEEN FOOTHILL TRANSIT AND THE LOS ANGELES COUNTY METROPOLITAN TRANSPORTATION AUTHORITY FOR THE AZUSA INTERMODAL TRANSIT CENTER LOCATION: 801 N. ALAMEDA AVENUE. RECOMMENDED ACTION: Approve a Parking License Agreement between Foothill Transit and Metro for the Azusa Intermodal Transit Center; and Authorize the City Manager to execute the agreement, in a form acceptable to the City Attorney, on behalf of the City. 4/1/2019 - 4 - 5. AUTHORIZATION TO SOLICIT A REQUEST FOR PROPOSALS FOR INSPECTION AND CLEANING OF CITY MAINTAINED CATCH BASINS. RECOMMENDED ACTION: Approve the RFP for services for inspection and cleaning of City maintained catch basins; and Authorize staff to solicit the RFP. 6. APPROVAL OF FIRST AMENDMENT TO AGREEMENT FOR MAINTENANCE SERVICES BETWEEN THE CITY OF AZUSA AND CONTROL AIR CONDITIONING CORPORATION AND CONSIDERATION OF INCREASING BLANKET PURCHASE ORDER. RECOMMENDED ACTION: Approve the First Amendment to Maintenance Services Agreement between the City of Azusa and Control Air Conditioning Corporation for HVAC system services; and Approve an increase to the blanket purchase order for Control Air Conditioning Services from $120,000 to $250,000; and Authorize the City Manager to execute the contract amendment, in a form acceptable to the City Attorney, on behalf of the City. 7. APPROVAL OF THIRD AMENDMENT FOR MAINTENANCE SERVICES AGREEMENT BETWEEN THE CITY OF AZUSA AND ULTIMATE MAINTENANCE SERVICES, INC. RECOMMENDED ACTION: Approve the Third Amendment to the Maintenance Services Agreement between the City of Azusa and Ultimate Maintenance Services, Inc. for City-wide janitorial services; and Authorize the City Manager to execute the contract amendment, in a form acceptable to the City Attorney, on behalf of the City. 8. AUTHORIZE THE CITY MANAGER TO EXECUTE A TASK ORDER FOR PROFESSIONAL SERVICES TO DEVELOP A PAVEMENT MANAGEMENT SYSTEM. RECOMMENDED ACTION: Authorize the City Manager to execute a Task Order with TKE Engineering, Inc. for a not-to-exceed fee amount of $108,500.00 to develop a Pavement Management System. 9. APPROVE THE PLANS AND SPECIFICATIONS AND AUTHORIZE STAFF TO SOLICIT BIDS FOR THE TRAFFIC MANAGEMENT SYSTEM PROJECT. RECOMMENDED ACTION: Approve the project plans and specifications; and Authorize Staff to solicit a Notice of Inviting Bids for the Traffic Management System Project. 4/1/2019 - 5 - 10. ACCEPTANCE OF PROJECT COMPLETION AND AUTHORIZATION TO FILE A NOTICE OF COMPLETION FOR THE COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) SIDEWALK AND ADA IMPROVEMENTS PROJECT NO. 601965-18 LOCATED WITHIN THE AREA BOUNDED BY FIFTH STREET ON THE NORTH, FOURTH STREET ON THE SOUTH, ORANGE AVENUE ON THE WEST, AND ANGELENO AVENUE ON THE EAST. RECOMMENDED ACTION: Accept the project improvements; and Authorize Staff to file a Notice of Completion for the CDBG-funded Sidewalk and ADA Improvements Project No. 601965-18. 11. AWARD OF CONTRACT FOR THE RE-PLASTERING OF CHILD AND SLIDE POOLS AT SLAUSON PARK AQUATICS CENTER LOCATION: 501 E. 5TH STREET. RECOMMENDED ACTION: Award contract to Condor, Inc, in the amount of $207,900, which includes a 10% contingency for potential change orders as needed, and Authorize the City Manager to execute a contract, in a form acceptable to the City Attorney, on behalf of the City. 12. APPROVE A MEMORANDUM OF UNDERSTANDING WITH VOLUNTEER CENTER OF SAN GABRIEL VALLEY. RECOMMENDED ACTION: Authorize the City Manager to execute the agreement, in a form acceptable to the City Attorney, on behalf of the City. 13. ACCEPTANCE OF “LUNCH AT THE LIBRARY” GRANT FROM THE CALIFORNIA LIBRARY ASSOCIATION. RECOMMENDED ACTION: Accept the grant award from the California Library Association; and Approve necessar y budget amendments and adjust the current fiscal year budget accordingl y. 14. ACCEPTANCE OF “VALUE OF LIBRARIES” GRANT FROM THE CALIFORNIA PUBLIC LIBRARY SERVICES BOARD. RECOMMENDED ACTION: Accept the grant award from the California Public Library Services Board; and Approve necessary budget amendments and adjust the current fiscal year budget accordingl y. 15. REQUEST TO SOLICIT BIDS FOR ADMINISTRATIVE AND LABOR COMPLIANCE PROFESSIONAL SERVICES FOR THE CITY OF AZUSA COMMUNITY DEVELOPMENT BLOCK GRANT PROGRAM. RECOMMENDED ACTION: Authorize staff to circulate a Request for Proposals (RFP) for Community Development Block Grant Administrative and Labor Compliance Services. 4/1/2019 - 6 - 16. FINDING OF PUBLIC BENEFIT TO THE COMMUNITY AT LARGE FOR RECOMMENDED EXPENDITURES APPROVING A REQUEST FROM GLADSTONE HIGH SCHOOL MATH CLUB IN THE AMOUNT OF $200. RECOMMENDED ACTION: Approve an expenditure in the amount of $200 to the Gladstone High School Math Club for the transportation; and Make a finding that the community at-large will receive a public benefit from the expenditure of funds for the recommended item. F. SUCCESSOR AGENCY RELATED BUSINESS. 1. RESOLUTION AUTHORIZING PAYMENT OF WARRANTS BY THE SUCCESSOR AGENCY DATED FEBRUARY 28, 2019 THROUGH MARCH 27, 2019. RECOMMENDED ACTION: Adopt Resolution No. 2019-R06 allowing certain claims and demands to be paid out of Successor Agency Funds. G. ADJOURNMENT.
