Valley Council of Governments.
B. Appointment of Cristina Madrid to the LA Work Board of Directors.

' for an Amount of $24,994.25. Recommendation: Approve a purchase order with Azusa
Wholesale Plumbing for fifteen (15) new park drinking fountains to replace non-ADA accessible
fixtures at various parks, in the amount of $24,994.25.

the amount of $20,546.93. Request to Enter into a Contract Agreement with Malibu Pacific
Inc. to Install a Picnic Shelter. BBQ and the Purchase of Picnic Tables, Concrete Pads and
Installation Labor in the Amount of$23.700.00 at Veteran's Freedom Park. Recommendation:
Approve the request for a purchase order to purchase a picnic shelter and BBQ from Sadie
Construction in the amount of $20,546.93. It is further recommended to enter into a Contract
Agreement with Malibu Pacific, Inc. to install said picnic shelter and BBQ and the purchase of
picnic tables, concrete pads and cost of installation labor in the amount of $23,700.00 at

Redevelopment Project Area. Recommendation: Open the Joint Public Hearing; receive
testimony, close the Hearing. Approve the Negative Declaration and adopt Resolution No. 03-
R8, approving the Amendment to the Disposition and Development Agreement dated May 6,
2002 ("DDA") between the City of Azusa Redevelopment Agency and Dr. Ralph Reyes and
Jeanine E. Reyes ("Developer").
