Subject: Change Order#33 for Police Facility Addition and Alteration Project
Background
The plans and specifications for the City of Azusa Police Facility Addition and Alteration Project
called for the replacement of our gasoline dispenser and pipe connection between it and the 10,000
gallon underground fuel storage tank(UST). Also, due to the lack of documentation verifying the
type of UST installed at the police facility in 1988,the possibility existed that the tank itself would
have to be replaced. The Environmental Protection Agency's(EPA)requires that all UST's comply
with the following: Corrosion protection, spill containment and overfill prevention requirements;
striker plates or drop tube-mounted bottom protectors; and secondary containment or tank lining.
The deadline to meet these requirements is December 22, 1998.
Findings
Fortunately,the UST at the police facility was found to be double walled and need not be replaced,
but only required a few modifications.However,although the contract with the Hazama Corporation
covers the replacement of the pipe connection and dispenser to comply with the EPA's standards,
the cost for removal of contaminated soils could not be anticipated. We did not encounter any
contaminated soils when the diesel UST was removed last year,but we were not as fortunate in this
case. Once the dispenser was removed,we were required to test the soils underneath and report the
findings to the Los Angeles County Department of Public Works. There was no contamination at
the actual tank site, but considerable contamination at under the dispenser.
The attached change order describes the allowance necessary to remediate the contaminated soils
and estimates a seven day delay in the project schedule.

Recommendations
Staff recommends that Change Order #33 be approved in for an amount not to exceed $17,881,
pending the approval of LaSalle National Bank(LNB).
