CORONAVIRUS (COVID-19) ADVISORY
Due to the state and local State of Emergency resulting from the threat of Novel Coronavirus (COVID-19),
Governor Newsom has issued Executive Order N-29-20 in which Section 3 supersedes Paragraph 11 of
Executive Order N-25-20 (issued on March 12, 2020) addressing Brown Act regulations. This new order states
that the City does not need to make a physical location available for members of the public to observe a public
meeting and offer public comment. The Order allows the City to hold City Council/Board meetings via
teleconferencing and allows for members of the public to observe and address the meeting telephonically or
electronically.
To comply with the recent Health Orders issued by the Governor and Los Angeles County Health
Department, the City Council meeting scheduled for Monday, November 2, 2020 at 6:30 & 7:30 p.m. are
Open to the Public for Public Comment only. Those wishing to comment on items virtually on the closed
session agenda 6:30 P.M. may do so by calling (669) 900-6833 and when prompted entering the following
meeting ID # 936 1866 1719 between 6:00 P.M - 6:30 P.M. Those wishing to make public comment virtually
on the Regular City Council meeting agenda 7:30 P.M. may do so by calling (669) 900-6833 and when
prompted entering the following meeting ID # 936 1866 1719 between 6:30 P.M. – 7:30 P.M. No calls will be
accepted after 7:30 P.M. Please note if you call in for Public Comment, you will be placed in a waiting room up
until your turn to speak. NOTE: You will not be able to hear the meeting live from the call but may access
the meeting by clicking the link below or on your local cable access channel. Members of the public may
also submit written comment by emailing PublicComment@azusaca.gov by 5:30pm. Emailed public comments
will be provided to the City Council and will be part of the record but will not be read aloud. (Email Comments
shall be limited to no more than 250 words).

NOTICE TO THE PUBLIC FOR REGULAR MEETING.
In compliance with Government code Section 54957.5, copies of staff reports or other written documentation
relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E.
Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police
Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.azusaca.gov. Persons who
wish to speak during the Public Participation portion of the Agenda or on a Public Hearing item, shall fill out a
card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted after will not
be accepted.

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items within the subject
matter jurisdiction of the council/agency board that are not listed on this agenda or are listed on this agenda
as an item other than a public hearing item. Members of the audience will have the opportunity to address the
city council/agency board about public hearing items at the time the public hearing is held. Under the
provisions of the Brown Act, the council/agency board is prohibited from taking action on oral requests, but
may refer the matter to staff or to a subsequent meeting. The council/agency board will respond after public
comment has been received. Each person or representative of a group shall be allowed to speak without
interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules.
Questions to the speaker or responses to the speaker's questions or comments shall be handled after the speaker
has completed his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS - CITY COUNCIL
1. CONSIDERATION OF ENTERING INTO A LICENSE AND REIMBURSEMENT AGREEMENT
WITH THE MOUNTAIN COVE COMMUNITY ASSOCIATION AND AUTHORIZE CITY
MANAGER AND CITY ENGINEER TO PROCURE MATERIALS/ENTER INTO A
CONSTRUCTION AGREEMENT TO IMPLEMENT MITIGATION EFFORTS.
RECOMMENDED ACTION:

1) Authorize the City Manager to enter into a license and reimbursement agreement with the Mountain
Cove Community Association in a form acceptable to the City Attorney.
2) Authorize the City Manager and City Engineer to procure materials/enter into a construction agreement
(in a form acceptable to the City Attorney) as needed to implement mitigation efforts recommended by
the United States Department of Agriculture (USDA) Natural Resources Conservation Service (NRCS).
2. AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF AZUSA, CALIFORNIA, ADDING
ARTICLE XXIV TO ARTICLE 18 AZUSA MUNICIPAL CODE, IMPOSING BUSINESS
REGULATIONS ON TOBACCO RETAIL OUTLETS.
RECOMMENDED ACTION:
1) Waive further reading, read by title only, and introduce Ordinance No. 2020-08; AN ORDINANCE OF
THE CITY COUNCIL OF THE CITY OF AZUSA, CALIFORNIA, ADDING ARTICLE XXIV TO
ARTICLE 18 AZUSA MUNICIPAL CODE, IMPOSING BUSINESS REGULATIONS ON
TOBACCO RETAIL OUTLETS
E. CONSENT CALENDAR – CITY COUNCIL/SUCCESSOR AGENCY
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.
1. MINUTES OF THE REGULAR MEETING OF OCTOBER 19, 2020.
RECOMMENDED ACTION:
Approve minutes as written.
2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED
OCTOBER 15, 2020 THROUGH OCTOBER 26, 2020.
RECOMMENDED ACTION:
Adopt Resolution No. 2020-C66 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.
3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service
Rules and applicable Memorandum of Understanding(s).

4. APPROVAL OF AN INTERNSHIP AFFILIATION AGREEMENT WITH CLAREMONT
GRADUATE UNIVERSITY.
RECOMMENDED ACTION:
Approve a Domestic Internship Affiliation Agreement with Claremont Graduate University to provide
Public Health Interns for All in for Azusa; and Authorize the City Manager to execute an Agreement with
Claremont Graduate University, in a form acceptable to the City Attorney, on behalf of the City.
5. APPROVAL OF A PROFESSIONAL SERVICES AGREEMENT WITH COACHMAN MOORE &
ASSOCIATES FOR ALL IN FOR AZUSA PROJECT MANAGEMENT SERVICES.
RECOMMENDED ACTION:
Approve a Professional Services Agreement with Coachman Moore & Associates for All in for Azusa
Project Management Services, in an amount not to exceed $32,000; and Authorize the City Manager to
execute an Agreement with Coachman Moore & Associates, in a form acceptable to the City Attorney, on
behalf of the City.
6. RESOLUTION TO APPROVE THE UPDATED CITY OF AZUSA TITLE VI PROGRAM.
RECOMMENDED ACTION:
Adopt Resolution No. 2020-C67 approving the updated City of Azusa Title VI Program, a requirement to
comply with the Civil Rights Act of 1964 for all agencies receiving Federal Transit Administration grant
funds.
7. ACCEPTANCE OF “CRISIS COLLECTION” GRANT FROM THE SOUTHERN CALIFORNIA
LIBRARY COOPERATIVE.
RECOMMENDED ACTION:
Accept the Crisis Collection Grant award from the SCLC; and Approve necessary budget amendments and
adjust the current fiscal year budget accordingl y.
8. APPROVAL OF AMENDED AND RESTATED MEMORANDUM OF UNDERSTANDING
BETWEEN THE CITY OF AZUSA AND THE CIVILIAN ASSOCIATION OF POLICE
PERSONNEL (CAPP).
RECOMMENDED ACTION:
Approval of amended and restated Memorandum of Understanding between the City of Azusa and the
Civilian Association of Police Personnel.
9. PURCHASE OF PAD-MOUNTED AND POLE-MOUNTED TRANSFORMERS.
RECOMMENDED ACTION:
Approve the issuance of a purchase order to ERMCO in an amount of $75,348.16 for the purchase of Pad-
mounted and Pole –mounted Transformers.

10. AWARD CONTRACT FOR COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG)
SIDEWALK AND ADA IMPROVEMENTS PROJECT NO. 602093-19, LOCATIONS CERRITOS
AVENUE BETWEEN HANKS STREET TO DUELL STREET, AND FIRST STREET FROM
PASADENA AVENUE TO RODECKER DRIVE.
RECOMMENDED ACTION:
Award a construction contract in the amount of $127,677, which includes 10% for contingency for potential
change orders as needed, to Kalban Inc. for the Sidewalk and ADA Improvements Project No. 602093-19;
and Authorize the City Manager to execute a contract, in a form acceptable to the City Attorney, on behalf
of the City.
11. AUTHORIZATION TO RELEASE A REQUEST FOR QUALIFICATIONS FOR ON-CALL
APPRAISAL SERVICES.
RECOMMENDED ACTION:
Authorize Staff to release a request for qualifications for on-call appraisal services from qualified firms
subject to non-substantive changes.
12. SUCCESSOR AGENCY APPROVE THE ASSIGNMENT AND ASSUMPTION AGREEMENT
ASSIGNING THE PURCHASE AND SALE AGREEMENT BETWEEN THE SUCCESSOR
AGENCY AND COSTANZO INVESTMENTS, LLC TO THE AVENUE AZUSA, LLC AND THE
CITY COUNCIL APPROVE THE ASSIGNMENT AND ASSUMPTION AGREEMENT
ASSIGNING THE DISPOSITION AND DEVELOPMENT AGREEMENT BETWEEN THE CITY
OF AZUSA AND COSTANZO INVESTMENTS, LLC TO THE AVENUE AZUSA, LLC AND
APPROVE THE FOURTH AMENDMENT TO THE DISPOSITION AND DEVELOPMENT
AGREEMENT. (APNs 8608-025-902, 8608-025-907, 8608-025-908, 8608-025-909, 8608-025-910 AND
A PORTION OF 8608-025-914).
RECOMMENDED ACTION:
The City Council approve, in substantially final form, an Assignment and Assumption Agreement
conveying Costanzo Investment’s interest in the DDA to The Avenue Azusa, LLC and approve in
substantially final form the Fourth Amendment to the DDA; and The Successor Agency approve, in
substantially final form, an Assignment and Assumption Agreement conveying Costanzo Investments
interest in the PSA to The Avenue Azusa, LLC; and Authorize the City Manager to execute the agreements,
in a form acceptable to the City Attorney, and to take any additional actions necessary to conclude the
transactions provided there is no additional cost to the City or the Successor Agency.
