This City Council Meeting will be in-person and open to the public, observing appropriate COVID-19 safety
precautions. All those attending in-person are required to follow posted guidelines. Those wishing to participate
during Public Comment may do so in-person at the Azusa Auditorium, or may submit written public comments
by emailing PublicComment@azusaca.gov. Emailed public comments are due by 5:30 p.m. and shall be limited
to no more than 250 words. Written public comments will be provided to the City Council and will be part of
the record but will not be read aloud.

1. Presentation of Proclamation proclaiming the Month of June 2022 as LGBTQ+ Pride Month in the City of
Azusa.

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items listed or not listed
on this Agenda. Members of the audience will have the opportunity to address the City Council/Agency Board
about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the
Council/Agency Board is prohibited from taking action on oral requests, but may refer the matter to staff or to a
subsequent meeting. Each person or representative of a group shall be allowed to speak without interruption
for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the
speaker or responses to the speaker's questions or comments may be handled after the speaker has completed
his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. PUBLIC HEARING ON PRELIMINARY PLAN REVIEW NO. 2022-01 FOR THE PROPERTIES
LOCATED AT 919 WEST SIERRA MADRE AVENUE (APNS: 8617-001-005, 8684-013-030, 8617-
001-013, 8617-013-001, 8617-011-001, 8684-043-002, AND 8684-013-014 - A REQUEST TO
DEVELOP EXISTING GOLF COURSE PROPERTY WITH SIX (6) INDUSTRIAL BUILDINGS,
250 - 300 RESIDENTIAL HOUSNIG UNITS, AND THE REDESIGN OF THE REMAINING
PROPERTY AS A NINE (9) HOLE GOLF COURSE.
RECOMMENDED ACTION:
Pursuant to policy adopted by the City Council in 2020, the City Council has two options as shown below.
Staff is seeking direction on Council’s preferred option.
1) Direct staff to continue processing the subject application.
2) Direct staff to discontinue processing the subject application.
Please note that a vote of the City Council to continue processing the subject application does not constitute
an approval of the project.
E. CONSENT CALENDAR – CITY COUNCIL
The Consent Calendar adopting the printed recommended actions will be enacted with one vote. If
Councilmembers or Staff wish to address any item on the Consent Calendar individually, it will be considered
under SPECIAL CALL ITEMS.
1. MINUTES OF THE REGULAR MEETING FEBRUARY 22, 2022 AND MARCH 7, 2022.
RECOMMENDED ACTION:
Approve minutes as written.

2. RESOLUTION AUTHORIZING PAYMENTS OF WARRANTS BY THE CITY DATED MAY 12,
2022 THROUGH MAY 30, 2022.
RECOMMENDED ACTION:
Adopt Resolution No. 2022-C39 allowing certain claims and demands and specifying the funds out of which
the same are to be paid.
3. HUMAN RESOURCES ACTION ITEMS.
RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
and applicable Memorandum of Understanding(s).
4. REQUEST TO INCREASE FISCAL YEAR 2021/2022 BLANKET PURCHASE ORDER FOR
FRONTIER COMMUNICAITONS.
RECOMMENDED ACTION:
Approve an increase to the Frontier Communications Blanket Purchase Order for Fiscal Year 2021/22 to a
not-to-exceed amount of $72,000
5. REQUEST TO INCREASE FISCAL YEAR 2021/2022 BLANKET PURCHASE ORDER FOR
GOVSENSE.
RECOMMENDED ACTION:
Approve an increase to the Govsense Blanket Purchase Order for Fiscal Year 2021/22 to a not-to-exceed
amount of $73,500
6. FOURTH AMENDMENT TO PROFESSIONAL SERVICES AGREEMENT BETWEEN THE
CITY OF AZUSA AND TRANSTECH ENGINEERS INC., TO PROVIDE CONTRACT PLAN
REVIEW, BUILDING OFFICIAL, AND INSPECTION SERVICES.
RECOMMENDED ACTION:
Approve the fourth amendment to the Professional Services Agreement with Transtech Engineers Inc. to
increase the term of their agreement through June 30, 2023 and adding Building Official services to the list
of those being offered to the City under said agreement; and Authorize the City Manager to execute the
amendment, subject to any non-substantive revisions approved by the City Attorney, and perform any
other obligations of the City under the amendment.
7. APPROVE BID RESULTS AND AWARD OF A THREE-YEAR CONTRACT TO MARIPOSA
LANDSCAPE INC. FOR CITYWIDE LANDSCAPE AND IRRIGATION MAINTENANCE OF
VARIOUS CITY PARKS, MEDIAN ISLANDS, PUBLIC FACILITIES, AND PUBLIC RIGHT OF
WAYS.
RECOMMENDED ACTION:
Approval the bid results received on April 20, 2022 for a three-year contract for landscape and irrigation
maintenance of various City parks, median islands, public facilities, public right of ways, and Azusa Light
and Water owned facilities; and Award a three-year contract to Mariposa Landscape Inc. for Landscape and

irrigation maintenance of various city parks, median islands, public facilities, public right of ways, and
Azusa Light and Water owned facilities.
8. ADOPTION OF RESOLUTIONS PERTAINING TO THE NOVEMBER 8, 2022 GENERAL
MUNICIPAL ELECTION AND REQUESTING THE BOARD OF SUPERVISORS OF THE
COUNTY OF LOS ANGELES TO RENDER FULL SERVICES TO THE CITY RELATING TO
THE CONDUCT OF A GENERAL MUNICIPAL ELECTION TO BE HELD ON TUESDAY,
NOVEMBER 8, 2022, WITH THE STATEWIDE GENERAL ELECTION TO BE HELD ON THE
DATE PURSUANT TO § 10403 OF THE ELECTIONS CODE.
RECOMMENDED ACTION:
Adopt Resolution No. 2022-C40, calling for the holding of a General Municipal Election to be held on
Tuesday, November 8, 2022, for the election of certain officers as required by the provisions of the laws of
the State of California relating to General Law Cities; Adopt Resolution No. 2022-C41, requesting the
Board of Supervisors of the County Los Angeles to render full services to the City relating to the conduct of
a General Municipal Election to be held on Tuesday, November 8, 2022; and Adopt Resolution No. 2022-
C42, adopting regulations for candidates for elective office pertaining to candidates statements submitted to
the voters at the General Municipal Election to be held on Tuesday, November 8, 2022.
9. FY 2021/22 THIRD QUARTER FINANCIAL REPORT ENDING MARCH 31, 2022.
RECOMMENDED ACTION:
Receive, review, and file the FY 2021/22 third quarter financial report ending March 31, 2022.
10. REQUEST TO APPROVE PROFESSIONAL SERVICES AGREEMENT WITH LDM
ASSOCIATES, INC. FOR ADMINISTRATIVE AND LABOR COMPLIANCE SERVICES FOR
THE CITY OF AZUSA COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) PROGRAM.
RECOMMENDED ACTION:
Approve a Professional Services Agreement with LDM Associates, Inc. for a three (3) year period for
Community Development Block Grant (CDBG) administrative and labor compliance services in an amount
not to exceed $40,000 for FY 2022-23; and Authorize the City Manager to execute the agreement, in a form
acceptable to the City Attorney, on behalf of the City.
11. APPROVE THE IMPROVEMENT PLANS AND SPECIFICATIONS FOR THE 2022
RESIDENTIAL ROADWAY REHABILITATION PROJECT AND AUTHORIZE STAFF TO
SOLICIT A NOTICE OF INVITING BIDS; PROJECT LOCATED WITHIN THE AREA
BOUNDED BY FOOTHILL BOULEVARD ON THE NORTH, FIRST STREET ON THE SOUTH,
ZACHARY PADILLA AVENUE ON THE WEST, AND SAN GABRIEL AVENUE ON THE EAST.
RECOMMENDED ACTION:
Approve the project plans and specifications; and Authorize staff to solicit a Notice of Inviting Bids for the
2022 Residential Roadway Rehabilitation Project.

12. INTEROPERABLE RADIO CONSOLE BASE STATION PURCHASE.
RECOMMENDED ACTION:
Waive the competitive bidding process as authorized under Azusa Municipal Code Section AMC Section 2-
523(e), contract #MA-IS-43070-10; Authorize the purchase of one Motorola MCC7500 Dispatch Console
Radio not to exceed $111,887; and Authorize the City Manager to execute the Purchasing Agreement, in a
form acceptable to the City Attorney, on behalf of the City.
13. SECOND AMENDMENT TO PROFESSIONAL SERVICE AGREEMENT BETWEEN THE CITY
OF AZUSA AND WILLDAN ENGINEERING INC. TO PROVIDE CONTRACT PLAN CHECK
REVIEW AND INSPECTION SERVICES.
RECOMMENDED ACTION:
Approve the Second Amendment to the Professional Services Agreement with Willdan Engineering Inc. to
provide plan review, building administration, and inspection services; Approve a one (1) year extension to
the Professional Services Agreement with Willdan Engineering Inc.; and Authorize the City Manager to
execute the Amendment, subject to any non-substantive revisions approved by the City Attorney, and
perform any other obligations of the City under the amendment.
F. ADJOURNMENT
We adjourn in solemn honor of those Azusans who we’ve lost to Covid-19.
