RECOMMENDED ACTION:
Approve the following Personnel Action Requests in accordance with the City of Azusa Civil Service Rules
and applicable Memorandum of Understanding(s).

2. REJECTION OF ALL BIDS AND PROPOSALS FOR DOG PARK CONSULTING DESIGN
SERVICES.
RECOMMENDED ACTION:
Reject all bids and proposals for the Dog Park Consulting Design Services.

3. APPROVAL OF AMENDED AND RESTATED FACILITIES USE AGREEMENT AMENDMENT
No. 3 FOR USE OF CITY FACILITIES INCLUDING MEMORIAL PARK LOCATED AT 320 N.
ORANGE PLACE FOR COVID-19 TESTING BY AND BETWEEN COUNTY OF LOS ANGELES
AND CITY OF AZUSA.
RECOMMENDED ACTION:
Review and approve the Amended and Restated Facilities Use Agreement Amendment No. 3 for use of city
facilities for COVID testing, by and between the County of Los Angeles and the City of Azusa; and,
Authorize the City Manager to execute the Amended and Restated Facilities Use Agreement Amendment
No. 3 for use of City Facilities, in a form acceptable to the City Attorney, on behalf of the City.

4. CONSIDERATION FOR RELEASING A REQUEST FOR PROPOSALS FOR POLICE TOW,
IMPOUND AND REFERRAL TOW SERVICES.

Authorize the City to release a Request For Proposals for Police Tow, Impound and Referral Tow Services.

5. AUTHORIZE THE CITY MANAGER TO EXECUTE AN EXCHANGE AGREEMENT OF
FEDERAL SURFACE TRANSPORTATION PROGRAM-LOCAL AND HIGHWAY
INFRASTRUCTURE PROGRAM FUNDS WITH THE LOS ANGELES COUNTY
METROPOLITAN TRANSPORTATION AUTHORITY.
RECOMMENDED ACTION:

Authorize the City Manager to execute an Exchange Agreement with the Los Angeles County Metropolitan
Transportation Authority to exchange the City’s Federal Surface Transportation Program-Local and
Highway Infrastructure Programs fund balances for local return dollars; and Appropriate $940,000 of
LACMTA funds for the Arrow Highway Median Installation Project and $239,242 of LACMTA funds for
the Foothill Boulevard Pavement Rehabilitation Project.

6. RE-APPOINTMENT OF MAYOR ROBERT GONZALES TO SAN GABRIEL VALLEY
MOSQUITO & VECTOR CONTROL DISTRICT.
RECOMMENDED ACTION:
Reappoint Mayor Robert Gonzales to The San Gabriel Valley Mosquito & Vector Control District for 2-
year appointment ending December 31, 2025.

7. ACCEPTANCE OF CAL OES GRANT FUNDING - HIGH FREQUENCY COMMUNICATIONS
EQUIPMENT PROGRAM.
RECOMMENDED ACTION:
Approve Mayor Robert Gonzales to accept the funding from CAL OES for High Frequency
Communications Equipment on behalf of the City; and Approve the purchase of High Frequency radio
equipment offered through the reimbursable CAL OES HF Communications Equipment Grant in an amount
not to exceed $60,000.

8. JUSTICE ASSISTANCE GRANT (JAG) - ACCEPTANCE OF GRANT FUNDING.
RECOMMENDED ACTION:

such funds pursuant to the terms and conditions of the U.S. Department of Justice.

TO THE MEMBERS OF THE CITY COUNCIL OF THE CITY OF AZUSA AND THE CITY CLERK:
NOTICE IS HEREBY GIVEN that a Special Meeting of the City Council is hereby called pursuant to Government
Code Section 54956, to be held on Monday, December 19, 2022, at 6:00 p.m., at the Azusa Auditorium, located at
213 N. Foothill Blvd, Azusa, California, for the purpose of considering the following:
