Prior to going into closed session, the City Council will convene in the Auditorium serving as Council

NOTICE TO THE PUBLIC FOR REGULAR MEETING.
In compliance with Government Code Section 54957.5, copies of staff reports or other written documentation relating to
each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E. Foothill Blvd.; copies
for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police Department Lobby - 725 N. Alameda
Ave., and the City of Azusa Web Page www.azusaca.gov. Persons who wish to speak during the Public Participation
portion of the Agenda or on a Public Hearing item, shall fill out a card requesting to speak and shall submit it to the City
Clerk prior to 7:30 P.M.; cards submitted after this time will not be accepted.
Those wishing to participate during Public Comment may do so in-person at the Azusa Auditorium, or may submit written
public comments by emailing PublicComment@azusaca.gov. Emailed public comments are due by 5:30 p.m. and shall be
limited to no more than 250 words. Written public comments will be provided to the City Council and will be part of the
record but will not be read aloud.

Notice Regarding California Environmental Quality Act (CEQA) Determinations. Pursuant to CEQA, a
"project" is defined as a "whole action" subject to a public agency's discretionary funding or approval that has
the potential to either (1) cause a direct physical change in the environment or (2) cause a reasonably
foreseeable indirect physical change in the environment. "Projects" include discretionary activity by a public
agency, a private activity that receives any public funding, or activities that involve the public agency's issuance
of a discretionary approval and is not statutorily or categorically exempt from CEQA. (Pub. Res. Code §
21065.) To the extent that matters listed in this Agenda are considered “projects” under CEQA, their
appropriate CEQA determination will be listed below each recommendation. If no CEQA determination is
listed, it has been determined that the action does not constitute a “project” under CEQA.

1) Proclamation proclaiming the week of May 21-27, 2023 as ‘National Public Works Week’ in the City of
Azusa.
B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items listed or not listed
on this Agenda. Members of the audience will have the opportunity to address the City Council/Agency Board
about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the
Council/Agency Board is prohibited from taking action on oral requests, but may refer the matter to staff or to a
subsequent meeting. Each person or representative of a group shall be allowed to speak without interruption
for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the
speaker or responses to the speaker's questions or comments may be handled after the speaker has completed
his/her comments. Public Participation will be limited to sixty (60) minutes.

D. SCHEDULED ITEMS
1. CONSIDERATION OF APPROVAL OF A SPECIAL EVENT PERMIT APPLICATION AND
WAIVER OF FEES BY AZUSA HIGH SCHOOL TO ALLOW FOR A PUBLIC FIREWORKS
DISPLAY IN CONJUNCTION WITH A COMMENCEMENT CEREMONY ON JUNE 6, 2023.
RECOMMENDED ACTION:
1) Consider and approve Azusa High School’s request for a Special Event Permit to allow for a one-time
firework display in conjunction with commencement activities on June 6, 2023;
2) Determine that the request is exempt from CEQA pursuant to Guidelines Section 15061(b)(3), also
known as the “general rule” exemption,
3) Authorize the City Manager to execute the Special Event Permit Agreement with Azusa High School in
a form acceptable to the City Attorney, on behalf of the City; and
4) Make a finding that the community at large will receive a public benefit from the expenditure of funds
for the recommended items and waive associated costs.

4. APPROVE SUBRECIPIENT AGREEMENT FROM 2022 STATE HOMELAND SECURITY
PROGRAM (SHSP).
RECOMMENDED ACTION:
Approve Mayor Robert Gonzales to sign the attached State Homeland Security Grant Program Subrecipient
Agreement Grant Year 2022 between the County of Los Angeles and the City of Azusa; and Approve the
purchase of Cybersecurity software and prevention resources offered through the reimbursable State of
California FY2022 Homeland Security Grant Program (SHSGP) in an amount not-to-exceed $45,577.

5. REQUEST TO INCREASE FISCAL YEAR 2022/2023 CONTRACT PURCHASE ORDER WITH J

PLANNING DIVISION.
RECOMMENDED ACTION:
Waive the competitive bidding process as authorized under Azusa Municipal Code Section. 2-517(a) (7) –
Reimbursement of Expenses; and Approve an increase of Blanket Purchase Order 038369 with J Lee
Engineering Inc. in the amount not-to-exceed $100,000.

7. APPROVE LETTER AGREEMENT WITH HELM & SONS AMUSEMENTS FOR OPERATIONS
AND SUPPORT SERVICES FOR AZUSA GOLDEN DAYS CARNIVAL.
RECOMMENDED ACTION:
Approve Letter of Agreement with Helm & Sons to operate a three-day carnival for weekend of October 20-
22, 2023; and Authorize the City Manager to execute the Letter Agreement, in a form acceptable to the City
Attorney, on behalf of the City, subject to non-substantive changes made by the City Manager and City
Attorney.

8. APPROVAL OF AN INTERNSHIP PARTNER AGREEMENT WITH CAL POLY, POMONA

Approve an Internship Partner Agreement with Cal Poly, Pomona to create an internship placement site in
the City of Azusa; and Authorize the City Manager to execute an Agreement with Cal Poly, Pomona, in a
form acceptable to the City Attorney, on behalf of the City.

10. AUTHORIZATION TO RE-INITIATE BID PROCESS FOR TENANT IMPROVEMENTS AND
PURCHASES RELATED TO CITY-OWNED FACILITY AT 890 THE PROMENADE
(PROMENADE AT CITRUS PROJECT).
RECOMMENDED ACTION:
Authorize Staff to solicit a Notice of Inviting Bids for construction services related to tenant improvements
of the Promenade at Citrus Project (CIP Project #62519A), subject to minor modifications; and Authorize
Staff to solicit a Request for Proposals for the purchase and installation of furniture, fixtures and equipment
(FF&E) related to the Promenade at Citrus Project (CIP Project #62519A), subject to minor modifications.

11. ACCEPTANCE OF GRANT FROM COMMUNITY PARTNERS TO IMPLEMENT THE CA
ACCOUNTABLE COMMUNITIES FOR HEALTH INITIATIVE (CACHI) THROUGH ALL IN
FOR AZUSA.
RECOMMENDED ACTION:
Accept the grant award from Community Partners/CA Department of Public Health; and Authorize the City
Manager to execute the Grant Agreement, in a form acceptable to the City Attorney, on behalf of the City,
subject to non-substantive changes made by the City Manager and City Attorney.
