SUBJECT: ACCEPTANCE OF GRANT FROM COMMUNITY PARTNERS TO
IMPLEMENT THE CA ACCOUNTABLE COMMUNITIES FOR HEALTH
INITIATIVE (CACHI) THROUGH ALL IN FOR AZUSA
BACKGROUND:
The City of Azusa has been awarded a grant in the amount of $300,000 from Community Partners,
which has an agreement with the California Department of Public Health, to fund thirteen (13)
sites across California who are implementing the CA Accountable Communities for Health
Initiative (CACHI). The City of Azusa, Community Resources Department, which serves as the
facilitator of All in for Azusa, is being awarded this grant to further expand this initiative work.
This action accepts this grant award and approves a corresponding budget amendment to
incorporate the grant funds into the appropriate fiscal year budgets.
RECOMMENDATIONS:
Staff recommends that the City Council take the following actions:
1)Accept the grant award from Community Partners/CA Department of Public Health; and
2)Authorize the City Manager to execute the Grant Agreement, in a form acceptable to the
City Attorney, on behalf of the City, subject to non-substantive changes made by the City
Manager and City Attorney.

ANALYSIS:
If accepted, the City of Azusa, Department of Community Resources would oversee this grant and
collaborative effort through All in for Azusa. This initiative has a Coordinating Council which
oversees the work conducted in Azusa, and currently has over 30 partners committed to bringing
services to Azusa residents. This initiative began as a pilot in 2020, led to the opening of the Azusa
Youth and Family Center, and continuously provides greater services for families. This funding
would support an expansion of services beyond the initial pilot location of the North Recreation
Center, to other areas in the City, but also expands recreational programming which will be
integrated into existing services. There is also a new aim to expand services that embrace the
City’s natural resources, such as the canyon, river, and recreational amentities, while promoting
stewardship and raising awareness of the rich history and cultural heritage of Azusa. This project
will continue to recruit new organizations and programs to Azusa, expanding and recreational
opportunities as well as family services.
FISCAL IMPACT:
There is no fiscal impact associated with the recommended actions. Upon Council acceptance
of the grant, Finance staff will amend the FY2022-23 and FY2023-24 budgets as appropriate
and consistent with the grant award amount and term which through 12/31/25.
