Archived video of recorded meetings is also available on the City website. Notice Regarding California Environmental Quality Act (CEQA) Determinations. Pursuant to CEQA, a "project" is defined as a "whole action" subject to a public agency's discretionary funding or approval that has the potential to either (1) cause a direct physical change in the environment or (2) cause a reasonably foreseeable indirect physical change in the environment. "Projects" include discretionary activity by a public agency, a private activity that receives any public funding, or activities that involve the public agency's issuance of a discretionary approval and is not statutorily or categorically exempt from CEQA. (Pub. Res. Code § 21065.) To the extent that matters listed in this Agenda are considered “projects” under CEQA, their appropriate CEQA determination will be listed below each recommendation. PUBLIC PARTICIPATION This time has been set aside for persons in the audience to make public comments on items listed or not listed on this Agenda. Members of the audience will have the opportunity to address the City Council/Agency Board about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the Council/Agency Board is prohibited from taking action on oral requests, but may refer the matter to staff or to a subsequent meeting. Each person or representative of a group shall be allowed to speak without interruption for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. CONTINUED PUBLIC HEARING FROM JANUARY 16, 2024 - APPEAL OF THE PLANNING COMMISSION’S DESIGN REVIEW DECISION TO ALLOW FOR THE EXPANSION OF AN EXISTING 110-ROOM HOTEL AND PERMIT THE ADDITION OF 23 NEW GUEST ROOMS, AND VARIANCE MODIFICATION REQUEST TO ALLOW FOR A REDUCTION IN PARKING SPACES AT 229 SOUTH AZUSA AVENUE, AZUSA CALIFORNIA. PRESENTATION OF FINAL DESIGN FOR OLD SCHOOLHOUSE RESTORATION AND HISTORIC ROW IMPROVEMENTS PROJECT AUTHORIZATION TO SOLICIT BIDS FOR CONSTRUCTION. 2/5/2024 - 4 - RECOMMENDED ACTION: 1) Authorize staff to solicit a Notice of Inviting Bids for construction services, to be prepared once construction drawings are completed in City’s standard form, related to the Old Schoolhouse Restoration and Historic Row Improvements Project (CIP Project Account 28-10-615-000/61524A) subject to minor modifications; and 2) Authorize Staff to solicit a Request for Proposals for the purchase and installation of furniture, fixtures and equipment (FF&E) related to the Old Schoolhouse and Historic Row Project. FOR CONSTRUCTION SERVICES; AND APPROVAL OF BUDGET AMENDMENT FOR THE AZUSA CITY LIBRARY OUTDOOR LEARNING SPACE PROJECT NO. #28-80-500-000/71122A (729 N DALTON AVE. AZUSA, CA 91702). CEQA - This action is categorically exempt from the requirements of the California Environmental Quality Act (CEQA) pursuant to Section 15301 Class 4 (b) – Minor Alternations to Land – New gardening or landscaping, including the replacement of existing conventional landscaping with water- efficient or fire-resistant landscaping. Staff has filed a Notice of Exemption with the Los Angeles County Clerk’s Office. RECOMMENDED ACTION: 1) Approve award of contract (CIP Project #28-80-500-000/71122A) to Leonida Builders, Inc. for construction services in the amount not to exceed $1,116,280 which includes a 10% contingency for potential change orders as needed; 2) Authorize the City Manager to execute the contract, in a form acceptable to the City Attorney, on behalf of the City; 3) Authorize the use of up to $567,270 from undesignated General Fund Excess Reserves (Account #4680510000/51024C) for construction and soft costs related to the Azusa City Library Outdoor Learning Space Project; 4) Authorize the use of $99,000 in Public Benefit funds (Fund-24) for energy efficiency-related project costs; 5) Authorize the use of $78,000 in unassigned Measure W funds (Safe, Clean, Water Program Fund-20) for qualifying project costs; 6) Authorize the use of $50,000 in Quimby funds (Park Fund-19) for park improvement-related project costs; 7) Authorize the use of $6,000 in AB939 funds (Fund-39) for mulch-related project costs; and 8) Authorize the use of $500,000 in California State Library Target Grant funds for this project. AWARD CONTRACT FOR THE COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) SIDEWALKS AND ADA IMPROVEMENTS PROJECT NO. 602600-23 FOR IMPROVEMENTS IN THE GENERAL AREA OF AZUSA AVENUE AND PARAMOUNT STREET. CEQA - This action is categorically exempt from the requirements of the California Environmentally Quality Act (CEQA) pursuant to Section 15301 Class 1 (c) – Rehabilitation of Existing Streets and Highways. RECOMMENDED ACTION: Award a construction contract in the amount of $113,520, which includes 10% for contingency for potential change orders as needed, to CT&T Concrete Paving, Inc. for the CDBG Sidewalk and ADA Improvements Project No. 602600-23; and Authorize the City Manager to execute the project contract, in a form acceptable to the City Attorney, on behalf of the City. 2/5/2024 - 7 - Total Cost: $103,200 Is the cost of this item budgeted? RECOMMENDED ACTION: Approve the contract documents and construction plans; and Authorize staff to solicit bids for Phases I through III for the Foothill Boulevard Pavement Rehabilitation Project. AUTHORIZATION TO ENTER INTO BLANKET PURCHASE ORDER AND WAVE PURCHASING PROCEDURES, SECTION 2-520(b) SOLE SOURCE OF THE AZUSA MUNICIPAL CODE TO ENTER INTO AN AGREEMENT WITH ACV LIGHTING CONSULTANTS, LLC TO PURCHASE 220 NEW LIGHTPOST LUMINAIRES THAT ARE SUBJECT TO THE DOWNTOWN STREETSCAPE IMPROVEMENT PLANS. RECOMMENDED ACTION: Approve a Blanket Purchase Order with ACV Lighting Consultants, LLC. to allow for the acquisition of 220 luminaires; Waive the competitive bidding process as authorized under Azusa Municipal Code Section2-502(b) which provides exceptions for sole source providers; and Authorize the City Manager to execute the Blanket Purchase Order, subject to any non-substantive revisions approved by the City Attorney, and perform any other obligations of the City under the PO. RECOMMENDED ACTION: 2/5/2024 - 8 - Authorize the City Manager to execute the MOA in a form acceptable to the City Attorney, on behalf of the City; and Authorize City Attorney/City Manager to make non-substantive changes to correct typographical errors or other document format edits.
