In compliance with Government Code Section 54957.5, copies of staff reports or other written documentation

Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police

wish to speak in-person during the Public Participation portion of the Agenda or on a Public Hearing item
shall fill out a card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted
after this time will not be accepted.
Those wishing to participate during Public Comment may do so in-person at the Azusa Auditorium, or may
submit written public comments by emailing PublicComment@azusaca.gov. Emailed public comments are due
by 5:30 p.m. and shall be limited to no more than 250 words. Written public comments will be provided to the
City Council and will be part of the record but will not be read aloud.

Notice Regarding California Environmental Quality Act (CEQA) Determinations. Pursuant to CEQA, a
"project" is defined as a "whole action" subject to a public agency's discretionary funding or approval that has
the potential to either (1) cause a direct physical change in the environment or (2) cause a reasonably
foreseeable indirect physical change in the environment. "Projects" include discretionary activity by a public
agency, a private activity that receives any public funding, or activities that involve the public agency's issuance
of a discretionary approval and is not statutorily or categorically exempt from CEQA. (Pub. Res. Code §
21065.) To the extent that matters listed in this Agenda are considered “projects” under CEQA, their

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items listed or not listed
on this Agenda. Members of the audience will have the opportunity to address the City Council/Agency Board
about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the
Council/Agency Board is prohibited from taking action on oral requests, but may refer the matter to staff or to a
subsequent meeting. Each person or representative of a group shall be allowed to speak without interruption
for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the
speaker or responses to the speaker's questions or comments may be handled after the speaker has completed
his/her comments. Public Participation will be limited to sixty (60) minutes.

1. CONTINUED PUBLIC HEARING FROM MARCH 4, 2024 - REQUEST TO APPROVE A
BILLBOARD RELOCATION AGREEMENT TO REMOVE AN EXISTING INOPERATIVE
DIGITAL READER OFF-PREMISE SIGN FROM 106 S. AZUSA AVE AND CONSTRUCT A NEW
DIGITAL BILLBOARD AT 250 E. 1ST STREET, AZUSA, CA, AND ADOPT RELATED
MITIGATED NEGATIVE DECLARATION.
RECOMMENDED ACTION:
1) Open the public hearing, receive testimony and closed public hearing;
2) Adopt Resolution No. 2024-C16; approving the Billboard Relocation Agreement and adopt related
Mitigated Negative Declaration and Mitigation and Monitoring Report for the project, and,
3) Authorize the City Manager to execute the Billboard Relocation Agreement and Temporary
Construction Easement, approve any nonsubstantive revisions, in a form acceptable to the City Attorney.

2. CONSIDERATION OF A SPECIAL EVENT PERMIT APPLICATION FROM ONELEGACY TO
CONDUCT A ONE AND FIVE KILOMETER RUN-WALK FAMILY FESTIVAL ON AND
AROUND THE AZUSA PACIFIC UNIVERSITY WEST CAMPUS AND THROUGH THE
ROSEDALE COMMUNITY IN THE CITY OF AZUSA.
RECOMMENDED ACTION:
1) Authorize the City Manager to execute the Special Event Permit Agreement for OneLegacy’s Donate
Life 1K/5K Run Walk, in a form acceptable to the City Attorney, on behalf of the City;
2) Authorize the City Manager to execute the Reimbursement Agreement for OneLegacy’s Donate Life
1K/5K Run Walk, in a form acceptable to the City Attorney, on behalf of the City; and
3) Adopt a Resolution No. 2024-C17 authorizing the closure of certain City of Azusa streets as discussed
herein for a limited time, in order to allow for the event to take place.

3. CONSIDERATION OF APPROVAL OF A SPECIAL EVENT PERMIT APPLICATION AND
WAIVER OF FEES FOR COMMUNITY EVENT ON MAY 4, 2024 IN CELEBRATION OF CINCO
DE MAYO.
RECOMMENDED ACTION:
1) Consider and approve the request for a Special Event Permit to allow for a one-time community boxing
event on May 4, 2024;
2) Authorize the closure of the public parking lot for the duration of the event;
3) Authorize the City Manager to execute the Special Event Permit Agreement in a form acceptable to the
City Attorney, on behalf of the City; and
4) Make a finding that the community at-large will receive a public benefit from the expenditure of funds
for the recommended items and waive associated costs.

4. AUTHORIZATION FOR THE PURCHASE OF OFFICE FURNITURE FOR THE AZUSA
POLICE DEPARTMENT.
RECOMMENDED ACTION:
Approve the Department’s request to purchase replacement office furniture in the amount of $84,915.00,
from D&R Office Works, Incorporated; and Approve the waiving of bidding requirements per Azusa
Municipal Code Section 2-521 Cooperative, piggyback and multiple awarded bid purchasing with other
agencies.

5. ACCEPTANCE OF FUNDING FOR 2023 CALIFORNIA OFFICE OF EMERCENCY SERVICES
HAZARD MITIGATION GRANT PROGRAM.
RECOMMENDED ACTION:
Authorize the City Manager to accept 2023 Cal OES Hazard Mitigation Grant Program in the amount of
$119,970, with a local match of $13,330, and execute any necessary contract(s) with the California
Governor’s Office of Emergency Service (Cal OES) and the Federal Emergency Management Agency
(FEMA).

6. ACCEPTANCE OF PROJECT SAFE NEIGHBORHOODS MINI-GRANT FUNDING FOR
$50,000.
RECOMMENDED ACTION:
Authorize the City Manager to execute all contracts, reports, and other documents required for receiving
such funds pursuant to the terms and conditions of the San Diego Association of Governments (SANDAG).

7. REQUEST TO RENEW MICROSOFT ENTERPRISE AGREEMENT WITH DELL INC. FOR
MICROSOFT PRODUCT LICENSING AND SOFTWARE ASSURANCE.
RECOMMENDED ACTION:
Waive formal sealed bids in accordance with Azusa Municipal Code Section 2-523 Section B, Computer
Software Maintenance; and Authorize the City Manager or his designee to approve a three-year renewal of
the Microsoft Enterprise Agreement with Dell Incorporated from May 31, 2024 – May 31, 2027 for
licensing and software assurance in the amount of $176,122.74 annually.

Approve a General Services Agreement between the City of Azusa and the County of Los Angeles for a
five-year period, commencing July 1, 2024 through June 30, 2029.

11. FILE NOTICE OF COMPLETION FOR THE ARROW HWY STREET RESURFACING AND
HIGHWAY SAFETY IMPROVEMENT PROJECT (PHASE I) AND THE ARROW HWY
LANDSCAPE IMPROVEMENTS PROJECT (PHASE II).
RECOMMENDED ACTION:
Accept the project improvements; and Authorize Staff to file a Notice of Completion for the Arrow
Highway Street Resurfacing and Highway Safety Improvements Project No. HSIPL-5112(019) – Phase I;
and Authorize Staff to file a Notice of Completion for the Arrow Highway Parkway & Median
Improvements Project – Phase II; and Authorize Staff to utilize the balance of the grant fund secured by
ALW ($44,367) to cover the drought tolerant and efficient irrigation systems constructed in Phase II.

12. AWARD A CONTRACT FOR THE FOOTHILL BOULEVARD PAVEMENT REHABILITATION
PROJECT – PHASE I.
RECOMMENDED ACTION:
Award a contract in the amount of $1,975,937.70, which includes a 10% contingency in the amount of
$179,630 for potential change orders as needed, to CT&T Concrete Paving Inc.; and Authorize the City
Manager to execute contracts, in a form acceptable to the City Attorney, on behalf of the City.

14. WAIVE THE COMPETITIVE BIDDING PROCESS AND AUTHORIZE: 1) THE PURCHASE OF
TWO FORD F-150 EV LIGHTNING TRUCKS VIA THE ENERGY EFFICIENCY AND
CONSERVATION BLOCK GRANT (“EECBG”) AND; 2) AUTHORIZE THE PURCHASE OF
ONE FORD MAVERICK HYBRID TRUCK VIA THE MOTOR VEHICLE SUBVENTION FUND
PROGRAM (“AB2766”).
RECOMMENDED ACTION:
Waive competitive bidding process as authorized under Azusa Municipal Code Section 2.521(a) and
approve the contract to purchase three vehicles through National Auto Fleet Group; Approve the purchase
of one 2024 Ford Maverick Hybrid by utilizing AB2766 Funds; and Authorize the City Manager or his
designees to utilize the U.S. Department of Energy EECBG allocation consisting of the City of Azusa’s
funding formula of $112,520 for the purchase of two 2024 Ford F-150 Lightning Trucks.
