In compliance with Government Code Section 54957.5, copies of staff reports or other written documentation
relating to each item of business referred to on the Agenda are on file in the Office of the City Clerk - 213 E.
Foothill Blvd.; copies for public view are at the Azusa City Library - 729 N. Dalton Ave., Azusa Police
Department Lobby - 725 N. Alameda Ave., and the City of Azusa Web Page www.azusaca.gov. Persons who
wish to speak in-person during the Public Participation portion of the Agenda or on a Public Hearing item
shall fill out a card requesting to speak and shall submit it to the City Clerk prior to 7:30 P.M.; cards submitted
after this time will not be accepted.
Those wishing to participate during Public Comment may do so in-person at the Azusa Auditorium, or may
submit written public comments by emailing PublicComment@azusaca.gov. Emailed public comments are due
by 5:30 p.m. and shall be limited to no more than 250 words. Written public comments will be provided to the
City Council and will be part of the record but will not be read aloud.

Notice Regarding California Environmental Quality Act (CEQA) Determinations. Pursuant to CEQA, a
"project" is defined as a "whole action" subject to a public agency's discretionary funding or approval that has
the potential to either (1) cause a direct physical change in the environment or (2) cause a reasonably
foreseeable indirect physical change in the environment. "Projects" include discretionary activity by a public
agency, a private activity that receives any public funding, or activities that involve the public agency's issuance
of a discretionary approval and is not statutorily or categorically exempt from CEQA. (Pub. Res. Code §
21065.) To the extent that matters listed in this Agenda are considered “projects” under CEQA, their
appropriate CEQA determination will be listed below each recommendation. If no CEQA determination is

B. PUBLIC PARTICIPATION
This time has been set aside for persons in the audience to make public comments on items listed or not listed
on this Agenda. Members of the audience will have the opportunity to address the City Council/Agency Board
about public hearing items at the time the public hearing is held. Under the provisions of the Brown Act, the
Council/Agency Board is prohibited from taking action on oral requests, but may refer the matter to staff or to a
subsequent meeting. Each person or representative of a group shall be allowed to speak without interruption
for up to five (5) continuous minutes, subject to compliance with applicable meeting rules. Questions to the
speaker or responses to the speaker's questions or comments may be handled after the speaker has completed
his/her comments. Public Participation will be limited to sixty (60) minutes.

1. A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF AZUSA APPROVING
PARTICIPATION IN THE LOS ANGELES URBAN COUNTY PERMANENT LOCAL HOUSING
ALLOCATION PROGRAM.

RECOMMENDED ACTION:
1) Open the public hearing, receive testimony and closed public hearing;
2) Review and adopt the following Resolution No. 2024-C26: A RESOLUTION OF THE CITY
COUNCIL OF THE CITY OF AZUSA, CALIFORNIA, APPROVING PARTICIPATION IN THE
LOS ANGELES URBAN COUNTY PERMANENT LOCAL HOUSING ALLOCATION PROGRAM
BY AUTHORIZING THE MAYOR, OR HIS/HER DESIGNEE, TO SIGN A REIMBURSABLE
CONTRACT WITH THE LOS ANGELES COUNTY DEVELOPMENT AUTHORITY (LACDA)
ACTING ON BEHALF OF THE COUNTY
3) Authorize the City Manager to execute the grant Agreement between the City of Azusa and the
California State Department of Housing and Community Development in a form acceptable to the City
Attorney.

2. AUTHORIZE LEASE AGREEMENT FOR THREE (3) ELECTRIC VEHICLES AS PART OF THE
MSRC LOCAL GOVERNMENT MATCH PROGRAM.
RECOMMENDED ACTION:
1) Waive competitive bidding as authorized under Azusa Municipal Code Section 2.521(a) and approve the
lease agreements with Enterprise Fleet Management for the lease of three (3)-2024 Nissan Leaf S 4dr
Hatchbacks through the use of a bid with TIPS-USA; and,
2) Authorize the City Manager to execute the lease agreements with Enterprise Fleet Management.

5. APPROVE LICENSE AGREEMENT WITH COLGATE BRIGHT SMILES, BRIGHT FUTURES
FOR ORAL HEALTH OUTREACH AND SCREENING FOR CHILDREN.
RECOMMENDED ACTION:
Approve License Agreement with COLGATE Bright Smiles, Bright Futures to provide oral health outreach
and screening visits, up to monthly, at City of Azusa Facilities; and Authorize the City Manager to execute
the License Agreement, in a form acceptable to the City Attorney, on behalf of the City, subject to non-
substantive changes made by the City Manager and City Attorney.

6. APPROVE LETTER OF AGREEMENT WITH AZUSA UNIFIED SCHOOL DISTRICT (AUSD)
FOR FINANCIAL SUPPORT OF SUMMER PROGRAMMING.
RECOMMENDED ACTION:
Approve a Letter of Agreement (Agreement) with the Azusa Unified School District to fund nine weeks of
summer programming for the 2024 summer, with an option to extend through the summer of 2025; and
Authorize the City Manager to execute the Agreement, in a form acceptable to the City Attorney, on behalf
of the City, subject to non-substantive changes made by City Manager and City Attorney.

7. ACCEPTANCE OF GRANT FROM U.S. DEPARTMENT OF AGRICULTURE (USDA) FOR THE
COMMUNITY CANOPY: NURTURING CLIMATE RESILIENCE IN DISADVANTAGED
NEIGHBORHOODS THROUGH URBAN FORESTRY PROJECT.
RECOMMENDED ACTION:
Accept the grant award from the U.S. Department of Agriculture; and Authorize the City Manager to
execute the Grant Agreement, in a form acceptable to the City Attorney, on behalf of the City, subject to
non-substantive changes made by the City Manager and City Attorney.

Equipment Use Policy.

9. ADOPT A RESOLUTION IDENTIFYING A LIST OF PROJECTS THE CITY PROPOSES FOR
FISCAL YEAR 2024-2025 FUNDED BY SB1: THE ROAD REPAIR AND ACCOUNTABILITY
ACT OF 2017.
RECOMMENDED ACTION:
Adopt Resolution No. 2024-C28 identifying the projects the City will fund with its apportionment of 2024-
2025 Road Maintenance and Rehabilitation Account; and Authorize staff to carryover existing RMRA funds
from the Gladstone Street Rehabilitation Project to 2023-24 Fiscal Year budget and CalSMART system.

10. AWARD OF A PROFESSIONAL SERVICES AGREEMENT FOR COMMUNITY
DEVELOPMENT BLOCK GRANT (CDBG) HOUSING REHABILITATION ADMINISTRATION
SERVICES TO FRJ & ASSOCIATES.
RECOMMENDED ACTION:
Award a professional services agreement in an amount not-to-exceed 20 percent of each housing
rehabilitation grant funded, up to a maximum amount of $37,000 annually, to administer the City's Single-
Family Housing Rehabilitation Project beginning July 1, 2024 through June 30, 2027; and Authorize the

City Manager to execute a professional services agreement, in a form acceptable to the City Attorney, on
behalf of the City.

11. AUTHORIZATION TO ISSUE A REQUEST FOR PROPOSALS ("RFP") TO SOLICIT BIDS TO

RECOMMENDED ACTION:
Authorize Staff to solicit a Request for Proposals to allow for the comprehensive update to the City of
Azusa’s General Plan, along with all necessary associated documents.

12. ADOPTION OF RESOLUTIONS PERTAINING TO THE NOVEMBER 5, 2024 GENERAL
MUNICIPAL ELECTION AND REQUESTING THE BOARD OF SUPERVISORS OF THE
COUNTY OF LOS ANGELES TO RENDER FULL SERVICES TO THE CITY RELATING TO
THE CONDUCT OF A GENERAL MUNICIPAL ELECTION TO BE HELD ON TUESDAY,
NOVEMBER 5, 2024, WITH THE STATEWIDE GENERAL ELECTION TO BE HELD ON THE
DATE PURSUANT TO § 10403 OF THE ELECTIONS CODE.
RECOMMENDED ACTION:
Adopt Resolution No. 2024-C29, calling for the holding of a General Municipal Election to be held on
Tuesday, November 5, 2024, for the election of certain officers as required by the provisions of the laws of
the State of California relating to General Law Cities; Adopt Resolution No. 2024-C30, requesting the
Board of Supervisors of the County Los Angeles to render full services to the City relating to the conduct of
a General Municipal Election to be held on Tuesday, November 5, 2024; and Adopt Resolution No. 2024-
C31, adopting regulations for candidates for elective office pertaining to candidates statements submitted to
the voters at the General Municipal Election to be held on Tuesday, November 5, 2024.

14. MEMORANDUM OF UNDERSTANDING (MOU) BETWEEN THE AZUSA CITY LIBRARY
AND IMMIGRATION RESOURCE CENTER OF SAN GABRIEL VALLEY (IRCSGV).
RECOMMENDED ACTION:
Approve the attached Memorandum of Understanding between the City of Azusa and the Immigration
Resource Center of San Gabriel Valley (IRCSGV) for continued citizenship preparation services through
the Azusa City Library.
