Notice Regarding California Environmental Quality Act (CEQA) Determinations. Pursuant to CEQA, a
"project" is defined as a "whole action" subject to a public agency's discretionary funding or approval that
has the potential to either (1) cause a direct physical change in the environment or (2) cause a reasonably
foreseeable indirect physical change in the environment. "Projects" include discretionary activity by a
public agency, a private activity that receives any public funding, or activities that involve the public
agency's issuance of a discretionary approval and is not statutorily or categorically exempt from CEQA.
(Pub. Res. Code § 21065.) To the extent that matters listed in this Agenda are considered “projects” under
CEQA, their appropriate CEQA determination will be listed below each recommendation. If no CEQA
determination is listed, it has been determined that the action does not constitute a “project” under CEQA.

A. PUBLIC PARTICIPATION
1. Person/Group shall be allowed to speak without interruption up to five (5) minutes maximum
time, subject to compliance with applicable meeting rules. Questions to the speaker or responses
to the speaker’s questions or comments shall be handled after the speaker has completed his/her
comments. Public Participation will be limited to sixty (60) minutes time.

1. Water Cost of Service Study Results and Authorization to Mail Notice of Public Hearing to
be Held October 23, 2023, for Water Rate Adjustment
Recommendation: 1) Approve the attached public hearing notice of the proposed water rate
adjustment to increase revenues by 15% in FY 2023/24, 9% in FY 24/25, and 4.0% annually in FY
2025/26 through FY 2027/28; and 2) Authorize Staff to print and mail the notice (pending final
legal review for verbiage) to all water customers 45 days before the public hearing to be held
October 23, 2023, at the regular Utility Board meeting time and place.

The Consent Calendar adopting the printed recommended action will be enacted with one vote. If
Staff or Councilmembers wish to address any item on the Consent Calendar individually, it will be
considered under SPECIAL CALL ITEMS.
1. Minutes of the Utility Board Meeting on July 24, 2023
Recommendation: 1) Approve Minutes as written.
2. Approve the Specification Drawing and Authorize the Solicitation of Bids for the Construction
of Underground Substructures Along Gladstone Street in Accordance with Project
LD2023-4
Recommendation: 1) Approve the plan and specifications for the underground line extension
Project LD2023-4; and 2) Authorize the advertisement and solicitation of bids for the construction
of LD2023-4, the underground line extension Project.
3. Approve Third Amendment to the Professional Services Agreement with Infosend, INC. to
Provide Utility Bill Print and Mailing Services
Recommendation: 1) Approve the Third Amendment to the Professional Services Agreement
(“Third Amendment”) with Infosend, in an amount not to exceed $275,000 annually, in accordance
with Azusa Municipal Code Section 2-517(a)(16); and 2) Authorize the Mayor to execute the Third
Amendment, including any related forms required by Infosend, in a final form acceptable to the
City Attorney.

4. Approve Second Amendment to the Professional Services Agreement with Invoice Cloud
Incorporated to Provide Electronic Bill Presentment, Payment and Credit Card Processing
Services
Recommendation: 1) Approve the Second Amendment to the Professional Services Agreement
(“Second Amendment”) with Invoice Cloud, in an amount not to exceed $275,000 annually, in
accordance with Azusa Municipal Code Section 2-517(a)(22); and 2) Authorize the Mayor to
execute the Second Amendment, including any related forms required by Invoice Cloud, in a final
form acceptable to the City Attorney.
