SUBJECT: APPROVAL OF THE NOTICE OF COMPLETION FOR UNDERGROUND LINE
EXTENSION IN ACCORDANCE WITH PLAN AND SPECIFICATION LD2023-4
BACKGROUND:
On September 23, 2024, the Azusa Utility Board approved a contract with Perry C. Thomas Construction
to furnish and install underground substructures in accordance with specifications for Project LD2023 -4
(Consent Item F-5). Staff has fully inspected the construction of this project and determined that it has
been completed within the specified timeframe and within the budgeted cost. Staff seeks the City Council's
acceptance of the Notice of Completion for Project LD2023-4 and authorizes the recording thereof in the
Office of the County Recorder of Los Angeles County.
RECOMMENDATIONS:
Staff recommends that the Utility Board take the following action:
1)Find that Perry C. Thomas Construction has completed the underground line extension with plans
and specifications for Project LD2023-4; and
2)Approve the Notice of Completion and authorize the Mayor to execute the Notice of Completion
on behalf of the City; and
3)Authorize the City Clerk to submit the Notice of Completion for recordation to the Office of the
County Recorder of Los Angeles County, within ten days of the Utility Board approval, as required
by Section 3093 of the California Civil Code.

ANALYSIS:
On September 23, 2024, the Azusa Utility Board awarded a contract to Perry C. Thomas Construction to
extend the electrical conduit and install four vaults along Gladstone St for Project LD2023-4 for
$1,531,762.05. On March 3, 2025, Perry C. Thomas began construction of the project and completed it
on July 17, 2025. Staff has fully performed the inspection of this project and determined that it has been
completed in accordance with the plan ED2023-4 budgeted cost amount.
Due to unknown field conditions, Staff has approved and authorized a couple of change orders for this
project. The total amount for the change orders is $74,695.70, which is 5.6% of the project base cost of
$1,331,967. The 15% contingency fund budgeted for this project was sufficient to p ay for the change
orders.
FISCAL IMPACT:
The project was completed on schedule and within budget, costing $1,404,162.70, including the cost of
the change orders. PO# 043124 has an available balance of $127,599.35. Funding for this project was
budgeted under the Electric Utility’s Capital Improvement Budget account no. 33-80-000-730-
7145/73017A-7145.
