The City Council of the City of Azusa met in regular session at the above date and time in the Azusa Auditorium
located at 213 E. Foothill Boulevard, Azusa, CA 91702.

The City Council reconvened at 7:30 p.m., City Attorney Martinez announced there was no reportable City Attorney
action taken in closed session. Reports

Presentation of Grant Awards by the Canyonty Foundation Board. Canyon City
Foundation
Mr. Mark Dickerson introduced the Foundation's Boardmembers; he gave a brief history of how the Board
Foundation was established; one of the conditions of the Development Agreement, was for the owners to Presentation of
set aside a portion of the proceeds from the sale of the property into a foundation to help support Azusa Grant Awards
community organizations and civic activities; each year the foundation receives applications from
various organizations requesting grants; they all have to be 501(c)(3) nonprofit organizations or a City of
Azusa department, and fall under IRS regulations; applications are accepted until the end of June and
grants are awarded by the end of November; prior to receiving applications, a letter of intent is sent to
community organizations to help them identify programs that may qualify for the grant; the bylaws call
for a Board of 5 members, 3 are selected by Monrovia Nursery and 2 by the City Council, for one year
terms; to date the foundation has granted $800,000, and this year $120,000 was granted. Councilmember
Carrillo added that the foundation has the ability to sustain itself from the ongoing income from the
resale of the properties.
Grants were presented to the following organizations for their approved programs:
1. City of Azusa Library, English as a Second Language (ESL) and other literacy programs;
'2. Azusa Unified School District (AUSD), summer programs;
3. Citrus College Foundation, Cafe Con Leche and the Ambassador Mentoring Programs;
4. The Neighborhood Homework House, Pre -Kindergarten Readiness and Parent Leadership programs;
5. AUSD, Keeping History Alive program.

Ms. Malinda Cruz, who donated he son's organs after his passing in 2010, invited Council to the Donate M. Cruz
Life Foundation event and fund raiser, on December 29, 2013, 3-5:00 p.m., at Pasadena Rosebowl, to Comments
place a rose on their float on behalf of City of Azusa.
Mr. Hector Elizalde, Military Liaison for Congresswoman Napolitano, announced that nomination H. Elizalde
applications will be accepted until November 29, 2013, for students wanting to pursue a career in one of Comments
the U.S. Military Academies, and a Health Care enrollment fair on December 14, 2013, at Lambert Park
Auditorium in EI Monte; and asked everyone to keep in mind the homeless Veterans.
Ms. Diana Williams, Human Resources Boardmember, and Mr. Michael Taylor, Executive Director for D. Williams
the Center for Education Diversity and Reconciliation, (C.E.D.A.R.), addressed Council thanking them Comments
for their continued support, and invited everyone to the Get To Know Me Forum Event, on December 5,
2013, 7:00 p.m., at the Azusa Auditorium.
Mr. Jorge Rosales addressed Council regarding Consent Calendar Item D-13, the Notice of Completion J. Rosales
for the Pavement Management Improvements Project #66113A, expressing concern that the streets were Comments
'opened for traffic prematurely before the seal adhered to the ground causing damage to the new
pavement.
City Manager Makshanoof addressed Mr. Rosales' concerns regarding the Pavement Management J. Makshanoff
Notice of Completion stating that the City Engineer and the Public Works Inspectors inspected the Comments
project and found it to be acceptable as finished.
REPORTS, UPDATES COUNCIL BUSINESS AND ANNOUNCEMENTS -STAFF
Rprts/Updates
Councilmember Carrillo wished everyone a Happy Thanksgiving, and reminded everyone that children A. Carrillo
will be on vacation the week of Thanksgiving. Comments
Councilmember Gonzales wished everyone a Happy Thanksgiving, and announced the Winter Fiesta on R. Gonzales
Sunday, December 1, 2013, 12-6 p.m., in front of City Hall; tree lighting will be at 5:00 p.m. Comments
Councilmember Alvarez thanked the Veteran's Committee for the event held on Veteran's Day, E. Alvarez
November 11, 2013; he wished everyone a Happy Thanksgiving; and congratulated the newly elected Comments
Boardmembers of the recent School Boards Election.
Mayor Rocha wished all a Happy Thanksgiving, announced that donations are being taken for the care J. Rocha
packets that will be sent to Azusa service men and women, on December 7, 2013, from the local VFW, Comments
and asked to keep service men and women in daily prayers.

Community Development Director Christiansen addressed the item explaining that this Public Hearing K. Christiansen
was scheduled at the meeting of October 21, 2013, to consider the amendment and re -adoption of the Comments
2013 Uniform Building Code, which is adopted every two years as mandated by the State of California.

Mayor Rocha declared the Hearing Open; City Clerk Cornejo read the affidavit of proof of publication Hrg Open
published in the San Gabriel Valley Tribune on October 31, and November 7, 2013. Public testimony
was sought by the Mayor, but none was received.
Moved by Councilmember Carrillo seconded by Councilmember Alvarez and *carried to close the Close Hrg
Public Hearing.

adopt Ordinance No. 13-03, by the following vote of the Council:

Introduction of Ordinance No 13-04, amending and restating Article III of Chapter 18 of the Azusa
Municipal Code to establish a regulatory scheme for administering, and managing the City's response to
alarms, fees, fines, appeal process, and providing other matters properly relating thereto.
Police Chief Gonzalez answered questions posed by Mayor Rocha explaining that this amendment is to
modernize the regulations for administering and managing the police alarm response; the current
ordinance was last updated in 1983, which has become antiquated due to changes in technology; at a
future meeting, staff will be presenting Council the schedule of the proposed fees that relate to the
various sections of this ordinance, as well as a proposal to outsource to the alarm management to a third
party; this update will have the same fee amounts for registration and renewal, provide discounts for
seniors, disabled and low income; it will be the responsibility of the alarm user to notify the alarm
company of any change in status, as the notifications to the Police Department will be monitored by the
company; the goal with this update is to reduce the number of false alarms that sometimes are caused
due to equipment malfunction; in order to reinstate the registration, all equipment requirements have to
be met, the company will educate customers on responsible alarm handling, as most of the false alarms
are due to owners not knowing how the system operates.

further reading and introduce, by title only, Ordinance No. 13-04 entitled:

The CONSENT CALENDAR consisting of Items D-1 through D-13, was approved by motion of Consent Cal,
Councilmember Alvarez, seconded by Councilmember Gonzales and *carried, with the exception of D-6 Spec Call
Item D-6, which was considered under the Special Call Portion of the Agenda.
The minutes of the special meetings of October 28, and November 7, 2013, and of the regular Minutes
meeting of November 4, 2013, were approved as written.

(DGS) — Military Department, for the use of the former Azusa National Guard Armory, now the Natl Guard
Memorial Park North Recreation Center (NRC), was approved.
8. The following Resolution No. 13-C88 was adopted ad entitled: Reso Grant

RESOLUTION OF THE CITY COUNCIL OF THE CITY OF AZUSA APPROVING THE Space
APPLICATION FOR GRANT FUNDS FROM THE LOS ANGELES COUNTY REGIONAL PARK
AND OPEN SPACE DISTRICT FOR FIRST SUPERVISORIAL DISTRICT EXCESS FUNDING
FOR THE FOLLOWING PROJECTS: Slauson Park Pool Rehabilitation, Zacatecas Park Restrooms
Development, Northside Park Walking Path Development, Pioneer Park, Memorial Park, Veteran's
Freedom Park and Gladstone Park Improvements.
9. The amendment to the current five year lease agreement with Plaza de la Raza Head Start, Inc., for Plaza de La

10.The 4'/, year lease agreement extension with Options Child Care, a child care and human services Option Child
agency, for use of the facilities at Memorial Park to conduct a Head Start Program was approved. Care Agmnt

reviewed and filed.
12.It was found that the project of the Final Tract Map No. 71774, for the property located at the Agreement
northeast corner of 9th St. and Alameda Ave., complies with the General Plan and is consistent with Tract Map
the approved tentative map and any amendments thereto; the Agreement for Completion of Public 71774, 9th &
Improvements for Tract No. 71774 to construct the required public improvements was approved and Alosta
the City Manager was authorized to execute the same; the Performance Bond in the amount of
$257,000, and the Labor and Materials Bond for $257,000, as guaranteed by International Fidelity
Insurance Company were accepted; it was found that this project is in compliance with the provisions
of the Subdivision Map Act; the Final Tract Map No. 71774 was approved; the City Clerk and
Treasurer were authorized to endorse the certificates on the face of the map, which embodies the
approval of Final Tract Map No. 71774.
13.The Notice of Completion for the Pavement Management Improvements Project #66113A — All NOC All
American Asphalt, Corona, CA 92878 was approved, and staff was authorized to file the Notice of American
Completion with the Los Angeles County Clerk. Asphalt

been available; staff has been going to conferences, meetings, working with other professionals in law
enforcement, to look at the many systems available from local to national companies; the resounding
reports heard back from all counterparts that manage these systems are that Spillman is the best; for the
past 15 years the Azusa Police Department has used Lucent Technology, which was bought off after
going bankrupt, a systems that is now only being used by Azusa and one other Police Department,
nationwide; Spillman has a far reaching solution that provides real time verifiable data that can be
instantly accessed; regarding big purchases that are common in law enforcement, if another agency has
satisfied the bid process, other law enforcement agencies can piggy back on the bid, because the
competitive bid process has been accomplished through the General Services Agreement (GSA) of the
California Multiple Award Schedule (CMAS).
City Attorney Martinez explained that in accordance with the software license agreement, the bid M. Martinez
'submitted to the bidding agency, will include all the various modules that the company provides, Address Item
allowing another agencies to pick and choose from those modules and piggy back on those contracts or
the original bid that was provided, as long as the price is the same or less than what it was given on the
bid; the Azusa Municipal Code allows for this process.
Further explanation was given on the multiple software demonstrations given by Spillman to all Azusa
Police personnel in different settings, and how the system was scrutinized by all staff, from police
officers on the streets to the office investigative bureau, and the report back from all staff was that they
were impressed with the system; in addition Azusa Lateral Police Officers that have worked at other
agencies that have this system have reported satisfactory findings.
Moved by Councilmember Alvarez, seconded by Councilmember Carrillo and *carried to approve the Approve
purchase of a Public Safety Software, to include Computer Aided Dispatch (CAD) Records Management Spillman Tec,
System (RMS) and Mobile Data Computer (MDC), from Spillman Technologies, Inc., in a total amount P.D. Public
of $688,122.00, and to authorize the Mayor to execute the Purchase and License Agreement, and the Safety
"piggyback" purchase in accordance with AMC Section 2-523(E) when the competitive bid process has Software
already been completed.
